Your GP surgery contributes to our meetings, providing vital medical information which can inform your care plan with the team. Once your keyworker has completed their assessment with you, they may have identified areas that need to be communicated back to your GP - this information can be discussed with your GP practice at our meetings (or earlier if required). This reduces duplication for you and ensures that all your needs are being met and communicated with all relevant professionals

One of these practitioners may be your keyworker if your referral needs are predominantly nursing or therapy related - they have the skills to complete a thorough assessment and will make all the necessary onward referrals to different services you may require input from and handover relevant information in our team meetings. The team aim to help keep people safe and well, out of social care and avoid hospital admissions, ensuring people live their best quality of life. They also link in with other services, make onward referrals and assess for equipment and minor adaptations. You may be given a keyworker from the Staying Well service if your referral needs are best met by the skills their team offers. Click the leaflet above for further info!

They can help you to create a plan to make life easier through one to one appointments and also offer drop in sessions - in a safe and warm space, where people can come along as they choose (without an appointment) to meet the social prescribers, connect with local people, services and organisations on your doorstep. 

If the team are unable to find the right support, they can help to set up groups and work with local partners to establish the support you need in your local neighbourhood. 

When referred into Active Case Management, your keyworker may be a social prescriber if their skills are best placed to meet your needs

There are many types of social care services including home care, day centres, care homes, end of life care and short-term reablement support. You may be allocated a keyworker from Adult Care if their skills best meet your referral needs

If you have ongoing input from services we have arranged, this will continue as long as it is necessary.

You will receive a feedback text message - this relates to the input you have received from our service/your key worker/s. If you have future needs that require input from Active Case Management, your health/social care professionals and GP can re-refer into our service
