Description of the Incident:

Time and Place: The incident occurred at 2:30 PM on March 23, 2023, in the production area of the ABC Manufacturing Company. 

Injuries: Unfortunately, his left hand got stuck in the conveyor belt.

Equipment: Conveyor belt

Detail: Tom had switched off the machine and was conducting routine checks on the conveyor belt. He found that one of the connections was bad. He started to change it when Tom turned the machine on. The conveyor belt moved and Tom's left hand got caught in it. His hand was seriously injured. There were no witnesses to the accident. However, I was working nearby, heard Tom's screams and ran to assist. When I saw Tom's wound, I ran to get a first aid box to loosely bandage Tom's wound. Then I called an ambulance to take Tom to the doctor.

Causes: An investigation that Tom had not been briefed on the above safety procedure by his immediate supervisor. When he was checking the machine, he didn't put up the "checking in progress" sign to let everyone around him know.

Recommendations: To avoid future incidents, it is recommended that the company implement a regular inspection and maintenance schedule for all production equipment, including the conveyor belt. Employees should also receive training on how to identify potential equipment malfunctions and report them promptly to their supervisors. Additionally, a system should be put in place to ensure all equipment is properly maintained and serviced according to the manufacturer's guidelines.
