         <description><![CDATA[<div>-Approximately 1 in 3 U.S. adults (about 75 million people) have hypertension (CDC, 2019)<br>-Only about half of these people have their elevated blood pressure under control (CDC, 2019)<br>-1 in 5 U.S. adults with hypertension are unaware they have it (CDC, 2019)<br>-Hypertension increases the risk for stroke and heart disease, which are some of the leading causes of death in the U.S. (CDC, 2019)<br>-Many youth are now being diagnosed with hypertension (CDC, 2019)<br>-Often, high blood pressure does not have any signs or symptoms which is why it is frequently referred to as the "silent killer" (CDC, 2019)<br>-At least 7 out of 10 U.S. adults with hypertension use medication to control their blood pressure (CDC, 2019)</div>]]></description>

These are available for health care professionals, policy makers, researchers and any individual looking for information on hypertension.          <description><![CDATA[<div><strong>NCHS Data Brief, Published October 2017</strong><br>-Provides data from the National Health and Nutrition Examination Survey<br>-Includes updated information from 2015-2016<br>-Trends from 1999-2000 through 2015-2016 are included as well <br><br><strong>Rationale:</strong> Discusses prevalence of hypertension by race, as well as control of hypertension<br><br><strong>Accessing Outcome Data:</strong><br>https://www.cdc.gov/nchs/products/databriefs/db289.htm<br><br><strong>Evaluating Outcome Data:<br></strong>-The overall prevalence of hypertension among adults in 2015-2016 was 29% (Fryar, Ostchega, Hales, Zhang &amp; Kruszon-Moran, 2017)<br>-The prevalence of hypertension among non-hispanic black adults (40.3%) was higher than non-hispanic whites (27.8%),  non-hispanic asian (25.0%), and hispanic (27.8%) adults (Fryar et al., 2017)<br>-48.3% of adults with hypertension in 2015-2016 had it controlled (Fryar et al., 2017)<br>-Hypertension control was higher among non-hispanic whites (50.8%) than non-hispanic black (44.6%) and Asian (37.4%) adults (Fryar et al., 2017)<br>-No significant change was seen in the percentage of adults with hypertension from 1999-2016, although the prevalence of controlled hypertension increased from 31.6% in 1999-2000 to 53.1% in 2009-2010-no significant changes were noticed from 2009-2010 to 2015-2016 (Fryar et al., 2017)<br><br><br><br></div>]]></description>

         <description><![CDATA[<div><strong>Strategy #1: </strong>Encourage participation in the Check.Change.Control Program<br><a href="https://www.heart.org/en/health-topics/high-blood-pressure/find-high-blood-pressure-tools--resources/check-change-icontroli-community-partner-resources">https://www.heart.org/en/health-topics/high-blood-pressure/find-high-blood-pressure-tools--resources/check-change-icontroli-community-partner-resources</a><br><br><strong>How reduces risks:</strong> Is an evidence-based hypertension management program that encourages self-monitoring of blood pressure to improve participants cardiovascular health; self-monitoring allows participants to closely keep tract of their blood pressure and avoid uncontrolled hypertension (Check, Change, Control, 2019).<br><br><strong>How improves health outcomes: </strong>Participants connect with a volunteer health mentor and share their progress (Check, Change, Control, 2019). Collaboration with others helps to keep participants on track and hold them accountable for monitoring their blood pressure.<br><br><strong>How preserves a healthy lifestyle: </strong><br> The program includes remote monitoring, online tracking, and recruiting local volunteer health mentors to encourage participants (Check, Change, Control, 2019). The use of remote monitoring and online tracking of blood pressure makes it very easy for participants to regularly stay on track with monitoring. It overall, assists participants to control their blood pressure and promote a healthy lifestyle. <br><br><strong>Strategy #2: </strong>Implement more community blood pressure screenings<br><br><strong>How reduces risks: </strong>Will allow individuals to have their blood pressure read and will identify if their blood pressure is elevated.<br><br><strong>How improves health outcomes: </strong>Many people are unaware they have hypertension; a convenient and local screening individuals are passing by in the community may be more appealing to people than having to go to their doctor's office. People with hypertension that are unaware of it are likely to find out their blood pressure is elevated at a screening.<br><br><strong>How preserves a healthy lifestyle: </strong>People with known hypertension can utilize a screening to see how well their blood pressure is controlled. Patients that are unsure about their blood pressure can obtain an accurate reading. Regular screenings will help people to stay on top of their current blood pressure. </div>]]></description>
