Communicate - speak clearly and if there are barriers to communication, I will write it down or find other ways to communicate non verbally such as hand gestures or eye movement for example blinking. I think communication is vital to build a relationship with the patient and taking a few minutes out of your shift to have a chat with the patient means the world to them as they can often feel lonely and secluded otherwise.
Choice - with their care, with meals, having a wash or shower are just a few examples. I feel that offering the patient a choice in every aspect helps them to feel involved in their care and what preferences they have, as they might like to do things in a certain way so this will help to keep their care individualised and specific to them.
Show dignity and respect - ensuring dignity and respect I think is very important as not everyone has the same beliefs. Maintaining a patients dignity should be a norm, such as when washing a patient, putting a towel over the patient and alternating washing different body parts at a time helps with this.

1. I make sure to introduce myself to the child and their family, and explain anything I am doing in simple terms so the child can understand - lots of children are very nervous coming in to hospital so involving them in their care and trying to help them understand can make them feel more in control and less anxious
2. I will try and chat to the child as much as possible and find out what is important to them (e.g. their favourite film/tv show, their favourite lesson at school, if they've got pets at home) to move away from a purely clinical relationship
3. As much as possible, I try to remain approachable and not to seem to busy to talk to, so the child - depending on their age- feels they can talk to me about any concerns or ask my any questions about their care that I can relay on to other members of the MDT

I read the patients documentation and any relevant care plans to learn more about their condition, needs and the way they prefer their care to be delivered. Patients can have a variety of preferences when it comes to care provision - for example a patient may prefer liquid medicine as opposed to swallowing tablets, they may prefer their OD medications at a certain time of day. Children may prefer parents to carry out tasks appropriate as opposed to the nurse.

2. I communicate before undertaking an intervention with a patient (or communicate with their parent when appropriate). I discuss the patients wants and needs when undertaking an intervention to allow minimal discomfort and emotional impact on wellbeing.

3. I communicate with other members of the MDT who may have looked after the patient for a long time to learn how to incorporate their holistic needs during care provision to enhance the quality of care delivery.

I believe that person centred care is central to improving the quality of care provided for the individual. I embed person centred care by:

1. Introducing myself, explaining my role and the purpose of my presence with the individual on that day. This I believe is very important, as in doing so, I highlight that the person in front of me is more than their diagnosis.

2. I always make sure to encourage the individual in my care to express any concerns, worries or doubts they may have regarding the care they are receiving. This is achieved by communicating therapeutically. When the patients express their worries, I actively listen to these concerns, using open-ended questions to find out more about the factors fuelling these worries as well as responding in a compassionate and empathetic manner. The consequence of this approach is the individual feeling more at ease, listened to and regarded as an equal and valuable partner in their care.

3. Finally, I understand that health is biopsychosocial in nature. This awareness allows me to prioritise the emotional and mental wellbeing of the individuals in my care as well as the physical tenets of health. Finding out the preferences, likes and dislikes, hobbies of patients elevates the emergence of care that not only takes these needs in to account but is also holistic.
