         <description><![CDATA[<div>From the Nurse<br>It was good keeping in touch with the patient and the Nurse was able to escalate care to other health professional.<br><br>The Nurse felt it was not successful because she could not access the patient's foot and diabetic machine since patient complained of foot injury and faulty reading on the blood glucose machine.<br><br>From the patient<br>She felt good being able to get in touch with someone since shielding for the past 2 months.<br>&nbsp;Patient was calm knowing that she has been referred to the district Nurse for further investigation.<br><br>The patient was worried being in contact with other health professional because she is in high risk category of contract COVID- 19. The patient was assured the District Nurse will be wearing her PPE. </div>]]></description>

         <description><![CDATA[<div>The communication device was not clear because the network signal kept dropping off.<br>The patient did not feel comfortable talking through the device. The Nurse had to assure the patient that the communication is confidential.<br>Some communication had to be repeated and delayed.&nbsp;<br><br>The Nurse missed the therapeutic touch to reassure the patient because the patient's moral was low due to the sound of her voice.</div>]]></description>

         <description><![CDATA[<div>The Nurse will use a numeric pain scale to assess the patient's pain.&nbsp;<br>Get the patient to describe the foot as much as possible to paint a picture for the Nurse to know if it is a point of concern.<br>Order a replacement blood glucose machine for the patient.<br>Refer the patient to district Nurse at her local community for further physical assessment of her injured foot, to get accurate blood glucose reading and health promotion for home exercise and meals of wheels (to improve her nutrition)&nbsp;<br><br></div>]]></description>
