  5. What is the pre/post data for each intervention cycle the student has participated in?
  6. What is the area of suspected disability?
  7. What accommodations are being used in the classroom?
  8. Is the student receiving any intervention from the classroom teacher?  If so, what is the pre and post data/info about what the teacher is targeting in these sessions to show growth or no growth?
  9. Are there any social/emotional concerns? If so, what tier 2 supports have been tried and for how long (e.g., check in/check out, sticks and stones, etc)?
