         <description><![CDATA[<p>we would use an incident report whenever something goes wrong on a site, like if someone were to get hurt, if equipment breaks, or even if there is a near miss that could've been serious. Basically, anytime safety is comprised or something unexpected happens that can affect the project. They document what happened, they help us find the cause, and show what we're doing to fix it. they also protect us legally and help improve safety moving forward. </p><p><br/></p><p><br/></p><p><br/></p><p>in such an environment the likelihood of workplace accidents increases and clear documentation is essential to understand what happened, who was involved, why it happened and how to prevent reoccurrence.</p>]]></description>
