Sore nipples are the common problem for most mothers who just started to breastfeed. It was due to the frequent latching in the first 2 weeks. In fact, you and your precious baby just started to work together and this is part and parcels of the breastfeeding journey. If your nipples are in severe pain, bleeding or cracked, you may apply nipple cream before and after latching to speed up the healing process. Alternatively, you can get the nipple shield when you latching and this tool may soothe the problem. More info: https://www.youtube.com/watch?v=b0fx-3-NkEY

Some other possible reasons behind sore nipples are poor latching, incorrect breastfeeding position, tongue tie or thrush infection. Thrush infection causes itchy and tender breast along with severe burning and sensations. All the other possible reason as stated above will require intervention by visiting lactation consultant or even pro-breastfeeding doctor as early as possible.

a. Engorgement usually happens around the 3rd day after birth, as a result of the milk coming in. With an older baby, it can happen any time when the baby is not feeding frequently enough. Breasts will feel very full, very heavy, warm to the touch and may also feel painful.

b. Start feeding on the fuller breast. Apply a warm compress a few minutes before feeding to allow the milk to flow. Express some milk to soften the areola. This will allow the baby to attach more easily. Let baby finish and drain the breast completely before offering the other breast. Feed as frequently as baby needs.

To prevent blocked ducts and mastitis, the breast needs to be emptied frequently and adequately. Hence, it is important that you feed baby frequently on demand (you may like to pump or hand express to relieve if you are feeling engorged in between feeds, especially if baby is sleeping longer stretches).

Sometimes, blocked ducts are also caused by wearing wired bras, wearing baby carriers that are too tight around the buckles or sleeping on a certain side. If this occurs, it will be best to latch baby as much as possible to clear the blockage as baby's suction is the most efficient in clearing the breast. Dangle feeding is also helpful sometimes. Other ways to relieve blocked ducts are hand expressing during a hot shower, using a wide toothed comb to massage the area, using a cold compress in between feeds or pumps to ease the pain.

Signs of mastitis include fever and chills that doesn't go away after 24 hours, red patches or streaks on the breast, swelling or hardening of the breast. If you are having any of these symptoms, do consult a doctor to get antibiotics to treat the infection as soon as possible.
