Here is one example of each that I've used in music:
Choice - last school year I led the students in a unit on Latin American music. They had a chance to choose a country to profile and locate a folk song to share with the class

Collaboration - Small group work in band to work towards polishing music in band

Communication - Using an LMS for students to track their progress.

Critical thinking - Peer teaching opportunities that promote reflection on 'how they know' so they can teach a concept to others

Creativity - use of improvisation and composition; flexibility of project choice

The 6th C - the music room is a place students come to 'hang out'; many informal moments.

-Neil

5 Cs in Biology

I would say I try to incorporate 3 out of the 5 Cs on a daily basis: Collaboration, communication, and critical thinking. Creative thinking I try to incorporate as well but that one is a work in progress. As for choice, I could do a better job as the only time the students have choice would be around questions/ topics for inquiry labs and concepts for project based learning.

Collaboration: Everyday the students are in Biology we do some sort of groups work where the students need to collaborate with their peers to try to understand a concept or work through a problem. I have twelve 3ft x 2ft white boards that the students use to get their ideas out.

Communication: Students in their groups are epected to share out what they come up with. Communication is a focus of our inquiry labs next year. After the students do their inquiry lab they will share their findings with their peers. The students put in a lot of time and effort into these labs so there should be more people reading them than just me.
Critical thinking is one of our primary focuses in our course and is one of the core competencies for the province. I do find it challenging to come up with meaningful lessons daily that are meant to challenge students ability to think critically about the given topic. The inquiry process is one way and also case studies and medical applications of the concepts are other ways to get students to go deeper

Here is a short example of how I incorporated the 4 out of the 5 Cs in my elementary English Class.

The assignment as a summer movie review based on a previous assignment.

Choice: Student were able to choose their own movie to do a character web and film analysis.

Creativity : Student were encouraged to pick the most significant scene and respond to it be either drawing it out in comic or writing a movie review.

Critical thinking : students were encouraged to try to put themselves in the main character’s shoes to justify the his or her action to resolve the story’s conflict.

Communication: students had to justify their choices and effectively present it to the class.

I think this project lacked collaboration part since students were usually working alone for the project.

“A guide on the side, rather than a sage on the stage.” - Joe Ruhl

As a math high school math teacher, I have consistently incorporated 4 of the 5 C’s through out different courses.

Choice

For all assignments, I give students a choice in how they want to present their work. This could take the form of a creative video, digital presentation, a drawing, a report, etc. I find that when students have choice, they are more engaged in the task. I remember being a student myself and having to hand in an assignment in a format that I wasn’t comfortable with. I would find this task daunting, time consuming and boring.

Collaboration/Communication

I simply love collaborative work! I provide students with as many opportunities to collaborate with their peers. It is important for students to recognize that learning isn’t linear and can take place in any form with anyone. Students participate in think-pair share, jig-saw, gallery walks, four corners, etc. It through these social activities that students are able to develop their communications skills.

- Zarah
