ثانياً: كيف يحدث المرض الوراثي؟
يحدث المرض الوراثي نتيجة للتغيّرات الجينيّة والتي يمكن أن تؤدّي إلى حدوث الطّفرات، وتُعرّف الطفرة بأنّها تغيُّر دائم في ترتيب القواعد (الحروف) في ال DNA الذي يكوِّن الجينات والذي يؤدّي إلى تغيُّر في وظيفة هذا الجين أو البروتين الذي ينتج عنه. وهناك نوعان من الطفرات:

النوع الأوّل:هي الطّفرات الوراثيّة التي يمكن أن تنتقل من الأب أو الأم أو كلاهما إلى الأبناء، ولذلك تتواجد في كل خلايا الجسم.
النوع الثّاني: هو الطّفرات الجسديّة التي تحدُث في بعض الخلايا أثناء حياة الإنسان نتيجة تعرُّضه لبعض العوامل البيئيّة، مثل التعرُّض إلى الأشعّة فوق البنفسجيّة أو للأشعّة النوويّة، وهذه الطّفرات هي التي يمكن أن تؤدّي إلى حدوث السرطان مثلاً.
وفي الحقيقة قد يحدث في بعض الأحيان أن يتغيَّر عدد الكروموسومات في الخلايا فقد تزيد أو تنقص، وبالتالي يؤدي ذلك أيضاً إلى بعض المشاكل والأمراض. ومن أشهر هذه الحالات وجود نسخة زائدة من كروموسوم رقم 21، وهذا الكرموسوم الزائد هو الذي يؤدّي إلى ما يُعرَف الآن باسم متلازمة داون وبالتالي ينتج عنه امتلاك الشخص المصاب 47 كروموسوماً في خلاياه بدلاً من 46 كروموسوم.

ثالثاً: العوامل التي تؤدّي إلى الإصابة بالأمراض الوراثيّة
يوجد هناك نوعان من العوامل بشكل أساسي يزيدان من احتماليّة ظهور الأمراض الوراثيّة وانتقالها إلى الأجيال اللاحقة، وهما:

العامل الأوّل (زواج الأقارب):
يزيد زواج الأقارب من فرص انتقال الأمراض الوراثيّة للأبناء، إذ ترتفع النسبة من 2% إلى 3% في حالات الزواج من غير الأقارب، لتصبح من 5% إلى 6% بين الأزواج الذين يرتبطون بروابط الدم. ومن أمثلة الأمراض المنتشرة والتي تنتج عن زواج الأقارب فقر الدم المنجلي وأنيميا البحر الأبيض المتوسط، ويزداد معدّل الولادة المبكّرة بين أطفال الأقارب مقارنةً بغيرهم من الأطفال.

وتختلف نسب زواج الأقارب باختلاف البلدان ففي الولايات المتحدة لا تزيد نسبة الزواج ما بين ابن وابنة العم أو الخال عن واحد في الألف وتبلغ تلك النسبة معدلاً عالياً بين اليابانيين (أربع في كل مائة زيجة). ولقد أظهرت إحدى الدراسات نسبةً عاليةً جدّاً لزواج الأقارب في الجماهيريّة الليبيّة فمن دراسة حال خمسمائة امرأة تبيَّن أنَّ ثمانية وأربع في المائة منهنَّ متزوّجات من أقربائهن.

العامل الثّاني (يتعلّق بأنماط الحياة):
حيث أنَّ العوامل التي تتعلق ببعض أنماط الحياة مثل التدخين والنظام الغذائي غير الصحّي (وخاصة الغذاء الذي يحتوي على نسبة عاليّة من الدهون) يزيد من فرص حدوث تغيُّر في بُنية العديد من الجينات، وبالتالي ظهور بعض الأمراض الوراثيّة.

رابعاً: أنواع الأمراض الوراثيّة
1- الأمراض الجينيّة السائدة:
إنَّ بعض الجينات التي نرثها من والدينا تكون أقوى من الأخرى. فنقرة الحنك يقال عنها بالمتغلِّبة ويحدِّد ذلك بالطبع جينات خاصّة غير ضارّة، إلّا أنَّ العديد من الجينات المتغلِّبة أو السائدة تؤدّي إلى أمراض خطيرة. فإذا ما حمل أحد الأبوين جيناً منها فسينتقل هذا الجين إلى نصف أطفاله (وهذا يعتمد على الاحتمال. فعندما نقول نصف الأطفال نعني أنَّ احتمال ظهور طفل مصاب هو ٥٠% وليس ذلك شرطاً لظهور طفل مصاب).

وتنتقل هذه الأمراض من جيل إلى آخر، أي من الأجداد، إلى الآباء، إلى الأبناء، وهناك حوالي 600 مرض يُورّث بهذهِ الطريقة، وتتميّز هذه الأمراض بالعديد من الصفات منها:

الأمراض السائدة تعني أنَّ وجود مورِّثة واحدة من أحد الوالدين كافيّة لظهور المرض.
ينتقل المرض عند الطفل الذي انتقلت له هذه المورِّثة، لذلك تظهر الحالة في كل الأجيال بشكل متتالي.
إنَّ حامل المرض وهو أحد الأبوين لا يُعاني أعراضاً ظاهرة للمرض.
قد يحمل المورّثة أحد الجنسين الذكور أو الإناث.
من أهم الأمراض التي تورَّث بصفة جسميّة سائدة هو داء فون ريكلينغهاوزن وداء هيتنغتون.
2- الأمراض الجينيّة المُتنحّية:
وهي الأمراض التي يُشترط للإصابة بها وراثة الجين الذي يوجد به مشكلة من كِلَا الأبوين، ويقصد بكلمة متنحّية أنّها بحاجة لمورِّثة من كل من الأب والأم لكي تسبّب المرض. وعند وراثة جين واحد به الخلل من أحد الوالدين فسيكون الشخص حاملاً للمرض وليس مصاباً به مثل مرض (الفينيل كيتون يوريا) والذي يؤدّي إلى تلف الدماغ ومضاعفات خطيرة أخرى. كما يوجد هناك حوالي 500 مرض يورّث بهذهِ الطريقة، وتتميّز هذهِ الأمراض بعدة صفات نذكر منها:

وجود مورّثة واحدة تسبّب حالة تسمّى بحامل للمورّثة أو حامل للمرض ولكنّه غير مصاب.
قد يحمل المورّثة أحد الجنسين الذكور أو الإناث.
من خصائص الأمراض الوراثيّة المتنحّية استطاعة جيناتها الانتقال من جيل إلى آخر والديمومة بدون ظهور أي حالة مرضيّة في تلك العائلة. فقد تغيب لتعود وتظهر فقط عند اجتماع حملة المورّثات.
هناك نسبة 25% أن يولد الأطفال دون أي إصابة، وهناك نسبة 25% من الأطفال يولدون حاملين للمرض وتظهر عليهم الأعراض.
3- الأمراض الكروموسوميّة:
وهي الاضطرابات الوراثيّة التي تنتج عن التغيّر في عدد أو بنية الصبغيات (الكروموسومات). ومن هذه الأمراض الوراثيّة الناتجة عن ذلك، متلازمة داون ومتلازمة تيرنر ومتلازمة كلاينفلتر. وتعود حوالي 20% من حالات الإجهاض إلى التشوهات الكروموسوميّة، حيث يزيد من احتمال حدوث هذه التشوّهات عديد من العقاقير والأدوية الكيميائيّة.

وتنقسم هذه الأمراض إلى نوعين، هما:

تغيُّرات عدديّة: حيث ترجع الكثير من الأمراض والعاهات إلى تغيّرات تحصل في عدد الكرموسومات كزيادة أو نقص كروموسوم كامل أو جزء صغير من الكروموسوم. فتظهر على الفرد نتيجة هذه التغيّرات الكروموسومية تغيرات معينة في صفاته الخارجية. والتشوّهات من هذا القبيل كثيرة ومتنوّعة وتختلف نسبة انتشارها من مجتمع لآخر. كالإصابة مثلًا بمرض متلازمة داون والذي يكون فيه المريض مغولي الوجه ويعاني من التخلّف العقلي، بالإضافة إلى تغيّرات وتشوّهات فيزيولوجيّة وجسميّة عديدة.
تغيُّرات بنيوية: وهي الأمراض التي تنتج عن تغيير في شكل وبنية الكروموسومات أو طبيعتها، وتصل نسبة هذه الأمراض إلى حوالي 5.6 لكل ألف مولود من الأحياء.
4- الأمراض المتعدّدة العوامل:
وهي الأمراض التي تكون نتيجة لأكثر من عامل وراثي وبيئي، وهذا النمط من الأمراض الوراثيّة لا ينجم عن خلل في مورّثة واحدة كما في الأنماط السابقة، بل ينجم عن الخلل في عدّة مورّثات أغلبها غير معروف حتى الآن.

وتسمّى باسم الأمراض الوراثيّة ذات الأسباب أو العوامل المُتعدّدة، ويندرج تحت هذا النوع كثير من الأمراض التي لم يُعرَف السبب الرئيسي لظهورها، أو الأمراض التي تتداخل فيها العوامل الجينيّة والبيئيّة، وتشمل عدداً من الأمراض أهمّها أمراض القلب، مرض السكري، السمنة، وارتفاع ضغط الدم، ومرض الزهايمر.
