In our first committee meeting, Both the Native Advisory Committee and the IHS spoke to huge disparities in data when it came down to rates of HIV in tribal/rural communities, and it wasn't clear who was responsible for the inaccurate data. Reading through the strategic plan, I don't get a sense of urgency for Native communities that I should for rates of HIV. I think this is a great section to include data related to substance/drug use in Indigenous communities as well and briefly highlight the epidemic here in order to showcase the heightened risk to Indigenous communities for HIV/STIs. Also, this is a great section to introduce, briefly, the problematic history of research and experimentation in Indigenous communities, and how it results still today in a lack of trust for outsiders, new medications, providers, etc. which leads (in part) to the resulting lack of data or accurate data reports mentioned previously. Because of this (and other reasons), many of the interventions outlined in other strategic plans or carried out in urban areas tend not to be as effective or applicable to Native communities. Then describe why our programming tend to be more 'promising practice' based. I'd also like to emphasize how the Tribal Health System exists to protect Indigenous people, and highlight how strong of a partner that Tribal Health Systems truly are. Indigenous people have been the 'test group' for Western experimentation and research too often. While we should still access the fact that there is up and coming research, it should not be so emphasized from my perspective. Highlighting proven methods that work to prevent HIV - condoms, abstinence, education, possibly PEP/PrEP, is my recommendation. Again, highlight Indigenous expertise - Community rural providers are the strongest allies when it comes to community-based or individual interventions.
