By establishing early relationships between ESRD patients and their care team, decreasing the number of emergent treatment starts is a realistic goal.

Vascular Access Champion - This person would be responsible for following the vascular access of each patient to ensure longevity of fistula.  Will require a strong working knowledge of the Dialysis Access Dashboard, and a strong working relationship with all referral sources.

Dedicated time away from direct patient care to allow time for access surveillance

Strong desire to be part of the education team of a chronically ill patient
