This is a transition planner. It's a graphic organizer I use to interview students who are over 15 to get an idea on their future plans. I interview them yearly until they graduate and reflect with parents on the changes. Students use this document to communicate to parents during IEP meetings about their goals and plans for their future.

I like this because we can use it for other students for the next year, and is not limited to just transitions. I love that you start in the middle and work out from there.

I don't have experience with any transitions but I have seen other teachers that I work with use career interest surveys to help students reflect on their future goals.

Since I have 11th, 12th, and +12th students, I find this personal assessment to be very helpful to develop transition goals and exit IEPs with both parent and student imput. Hope it helps!

I have only been a part of a transition meeting for my own daughter and just recently met with her current school IEP team and the new IEP team at the HS she will be attending to go over her accommodations and goals and also discuss her classes and overall plan.

I found this checklist that seemed easy to follow and really focuses on the student and what needs to be done at what age.

I haven't worked with Transitions, but I did find this template that would be great to use for students. It's an excellent way for students to develop leadership skills and determine their future goals independently.

I am looking now for something. Last year I taught 6th/7th SDC and this year 6th..so I have not had to do much in the way of transition, but I am looking for resources :-)

I do not have personal experience with transitions, but I know that some teachers use checklists and questionnaires for their students

John Flores Personal Preference Indicator(PPI)

The Personal Preference Indicators (PPI) help find out what a student likes by watching them and asking people who know them well. This helps teachers and families plan jobs and activities the student will enjoy. It is good for students who don’t talk much because it doesn’t need the student to answer questions. The PPI helps make goals that match what the student likes and helps them learn to make choices. It also helps plan for the student’s future work, living, hobbies, and support after school.

I currently a High School teacher and my district uses the COPS II Assessment to evaluate career and student interest. We also have the Transition Partnership Program to help students gain work opportunities and fill out college applications. They usually walk away from the program with complete enrollment to Copper Mountain Community College and continued employment with enrollment in Community College.

I work in early elementary and so this is not an area of an IEP that I regularly engage with. I however have a friend with an autistic high school age student and we discuss what is going on with his plans after high school.

There are job shadowing internships available in my area that seem to be very interesting! My friend’s son is very into art and is able to help out in a gallery in town. He even gets to display his work at different shows.

One of the transition planning assessments I am familiar with is the Transition Planning Inventory (TPI) which assists in identifying student strengths and preferences as it can relate to a possible career.
