She said Vickery was working in a different room from the apprentice when the accident happened.

The victim (who was 17 years old at the time) was doing live work on a surface socket outlet when he received a shock to his face from a live cable.

How to possibly have prevented the injury:

if the worker had been told all the WHS and OHS laws and policies to the apprentice then the apprentice would of had more knowledge being safe within the workplace and most probably would not have gotten shocked/another way would have been for the apprentice to be supervised when doing tasks.

Mr Atkinson, 59, climbed onto the step-deck trailer which was carrying a chock trailer to readjust one of the chains. 

The trailer had hydraulic oil on it and it was also raining. This caused Atakinson to slip off the trailer and fall on his shoulder. 
Mr Atkinson is suing Hogan's Heavy Haulage for $615,996.10 for personal injury and losses sustained as a result of the alleged breach of duty of care by his employer as they "failed to take reasonable care to avoid foreseeable risk of injury to Mr Atkinson while he was carrying out his employment duties", the claim said.

Solutions: better shoes, better training, slip-proof mats, towels to wipe up oil.

Why: A labourer standing on a loose insulation roll next to a skylight fell through to the concrete floor, suffering multiple fractures. The company was fined $70,000 and ordered to pay costs of $1369.

When: This happened on the 19 Dec 2014, 9:38pm.

Why: He fell into a collapsed trench at a building site because he didn't notice the trench and died at the scene. 

Prevent it: I recommend putting some tape around the area and signs to warn and or prevent people walking in.

Where: Downsview Park, Toronto, America 

When: June 17th, 2012 

How: Crew members ran away whilst overhanging staging and metal framework crashed on stage just before a Radiohead concert. The stage injured three workers when it collapsed an hour before the gates to the sold out Radiohead concert were opened. The weight of the roof was hastily approved by a structural engineer (i.e he was in a rush to get the stage set up as oppose to ensuring that the stage scaffolding can support the roof and framework).

When: 1^st of July 2017 

 

Incident:  A self-employed roof plumber died after falling off the roof of a multi-store apartment building. He had been the first at site to repair a leak on a roof. So that means no one was there to help him, he ended up being found out late. 

 

The roof plumber was working at height approximately 9 meters close to the edge there was a definite risk of falling to your death. The edge of the roof did not have adequate edge protection. The roof plumber was not using any kind of fall injury prevention system. 

 

 

How: A teacher at a school was walking to a different class room in between lessons. At the school there are walkways for people to move around the school. One of the walkways had been broken and not marked. The teacher tripped over the broken part of the walkway as it wasn't obvious that it was broken. As the teacher fell she landed on on her arm and broke her elbow.

Possible Solutions to prevent this situation:
*- Fix walkways before any dramatic injury can occur
*-Provide barriers around the broken walkway 
*-Better maintain the walkway so no brakes occur
