These substances can alter one's mind almost entirely, changing the way the brain's perception of things they way they interpret things, and the way they send things. This mostly has to do with the senses of the human body. For example, drugs such as cocaine can alter the neurons and effect the communication between body and brain. 
The reason people abuse drugs is to achieve a feeling of "relief and pleasure." How does thins work? Most drugs cause the brain to release more dopamine, which is a neurotransmitter that produces these feelings of pleasure. It therefore encourages the use of the drug(s).
