This was given to them in 1918. For combat camouflage uniforms, the insignia is turned to grey scale. Connection to the 28th infantry is linked back to the revolutionary war, where the militia called the "Associators" were assembled in 1747. The character Timmy in the 1968 film The Subject Was Roses was seen wearing his 28th division uniform throughout the movie. 
In both movies Dawn of the Dead and The Happening, soldiers in the film are seen wearing keystone patches on their uniforms. 
The division was also mentioned in the 5th episode of Band of Brothers.

Every third Sunday in May since 1919, members of the 28th Infantry Division have met for a memorial service at the shrine. The highway in which the shrine is located was named the Pennsylvania 28th Division Highway.
The 28th infantry has received multiple notable rewards including; the Presidential Unit Citation World War ll in 1944 for Battle of the Bulge,  French Croix De Guerre World War ll in 1944, the Meritorious Unit Commendation World War ll,  in 1945, and the Luxembourg Croix De Guerre, World War ll, in 1945. Ribbons for these awards are seen, in order, on the side.

America was neutral at this point in the war. 
Over seas, this division helped push East towards Paris, where they marched down the Champs-Elysees in 1944 while the Liberation of Paris was arranged.
They then headed to the German west wall, where the 28th infantry suffered mass casualties at the battle of Hurtgen Forest, which was one of the longest continuous battles of WWll. 
Due to the amount of causalities, the 28th was then assigned to a defensive position along the Meuse River until 1945, when they pressed steadily towards the city of Colmar. This led to the partial retreat of troops from Hitler.

They were ordered at Harrisburg in September 1950, with the outbreak of the Korean war. The 28th was mobilized to Europe under the NATO command in order to defend the world from te the threat of a soviet attack, until 1954. 
The division was not mobilized during the Vietnam War,  or Operation Desert Storm, but one of the infantry's constituent units served in Saudi Arabia and the Middle East. 
Some elements of the 28th served in Bosnia for many years after 1996 as peacekeepers, and in 2002, the division took command of the Northern Brigade Task Force, a part of the NATO peace keeping mission.
