I asked him why the work was incomplete and he was "too busy and tired to complete the assignment." 

Action: I said if you are too tired then you need to tell me. I gave an extension but decided not to contact the parents.

Incident: During class, the students were working on a group activity. Then, Heath and another student seemed to be in a heated discussion. Heath called the other student 'dumb'.
Action: I spoke with the student about the incident to see how their day had been and what had led to the incident. Heath shared that the student called him bald and it hurt his feelings.
Recommendation: No action is required. Monitor for now.
