This is a great formative assessment to use during the delivery of a lesson. It is a great way to keep track of whether or not the students are understanding what you are teaching. The students should all have a card on their desk with one green side and one red side. As you are teaching, if the students are fully understanding they should have their cards flipped on green. If at any time during your lesson a student does not understand or needs further clarification they simply flip their card over to red. When you see a students card on red it should be your cue to go back over in further detail until the student fully understands and is able to flip back to green.
https://www.prodigygame.com/blog/formative-assessment-examples/

Four Corners

Four corners is a great formative assessment to get the students up and moving. The teacher poses a question to the students, and gives them four answer choices. Each answer choice is represented as a corner in the room. These corners can be labeled as A,B,C,D to make it easier for the students. This will help give the teacher an understanding of how many students are understanding the content, while also making it a fun "game" for the students.
https://www.prodigygame.com/blog/formative-assessment-examples/

Exit Tickets

Exit tickets are a great formative assessment to evaluate how much students understood the content of a lesson. It is a great way to assess exactly what they may need extra work on. Especially if you were to provide a variety of exit tickets, and allowing students to pick the one with the content they specifically are struggling on. Each ticket gives students an opportunity to work on the skill they need to practice most. This is a great way of helping students become more self-aware and self-directed learners as they are taking the responsibility of practicing what they need work on. I believe this is a great concept for the older grades who are more self aware of the goals they individually need.
Source from Instagram: @rainbowskycreations

Turn it in bins

I love the idea of this formative assessment. The teacher has three different colored turn it in bins all labeled differently. The first is labeled as "I've got this", the second as "I almost have it, but I need some more practice" and the third is "I need some help, please reteach me!". I think this is a great way to get an understanding of whether or not the students understood something. The teacher can get a clear understanding of which students clearly understand the material or which students need more help. This can also help you determine whether or not you can pull a small group to reteach depending on the number you get in each bin.
source from instagram: @talesfromaverybusyteacher

This a very easy and quick formative assessment that can be quickly done at the end of a lesson. Its quick and truly gives you insight into what the students identified as the biggest ideas in the lesson. It is a great way to see if the students were able to take away the main points and ideas of the lesson. After reading these summaries the teacher should be able to gain knowledge for future lessons on what needs to be reviewed or retaught.
Source from Instagram: @fourthgradefrenzy

Round Robin Charts

Another great formative assessment is round robin charts. The students are placed into groups and charts will be passed around to assess understanding. Each group should be provided with a chart and markers in order to complete this activity. Each chart will contain an open ended question or a topic. The groups should write their answers and or information on their chart and pass it on to the next group. This rotation will continue until each group has gotten the chance to fill out each chart. At the end of the rotations the class will come back together as a whole and discuss the answers that were written on the charts. This is a great way for the teacher to get an understanding of the understanding of the group as well as to review or reinforce information that may have been missed.
https://globaldigitalcitizen.org/10-innovative-formative-assessment-examples

Think-Pair-Share

Think pair share is a great and simple way to formatively assess students. The teacher can ask the students a question and have them write their answer. Once they have their answers written down, they can pair up with a classmate and discuss their answer. The teacher can then circulate the room and listen in on discussions. It is a great way for teachers to get an insight on the level of understanding. This can also be done in the middle of the lessons to give the students a chance to let their wiggles out and talk to the person next to them. The teacher always has the chance to listen in and understand the students' thinking.
https://globaldigitalcitizen.org/10-innovative-formative-assessment-examples

This is a great formative assessment for the upper grades, as it will probably be seen as fun to them. This allows the students to "tweet" what they understand or don't understand at the end of the day. The students can be given the cards pictured where they can write out and turn in what they either don't understand, need more help on, or are 100% confident in. This could also be done by allowing the students to actually tweet if they have access to twitter and devices. This gives the teacher a clear understanding on the material that they may need extra help on, and is a fun way for the students to express themselves.
http://teachertakers.blogspot.com/2013/09/formative-assessment-before-you-go.html

Kahoot is another fun and engaging way to formatively assess students. The teacher can create a Kahoot with review questions and go through it with the class. The class can go through the questions and answer them all at the same time. This is usually seen as a fun review game to the students, and the teacher is easily able to assess what may need to be reviewed or worked on for the group as a whole.

Weekly Learning Reflection Log

This a great formative assessment to wrap up the week. The student is given a log that gives them the opportunity to sum up their week. The students use the boxes to write a brief description of what they learned in each class for the week. Then, the students are able to write their favorite part of the week, set a goal for the upcoming week, and they have an opportunity to tell the teacher anything they want to share. This gives the teacher a great understanding of what the students took away from that week, and it gives the teacher the weekend to plan out lessons according to what they observed from these reflection logs.
https://www.bloglovin.com/blogs/teaching-with-blonde-ambition-3782746/student-reflection-learning-3047112997
