The Wellness Specialist will assess the overall health and wellness of Endeavor Health Services clients. The successful candidate should be familiar with and monitor social determinants of health and address areas of need through education and coaching on an individual and group level. The Wellness Specialist will provide linkage to prescribers and community resources to assist the client in meeting their individualized goals.  This is a part-time position (up to 20 hours per week) evenings and Saturday mornings. 

Responsibilities:
  • Assess individuals needs and any potential gaps in care utilizing social determinants of health.
  • Creates and adapts person centered objectives to support individuals with overcoming barriers to their overall wellbeing.
  • Provides individualized coaching to clients in obtaining their identified objectives.
  • Facilitates groups/classes to support improved outcomes for clients.
  • Supports clients in learning how to make good choices for themselves.
  • Promotes individuals engagement with all necessary services and linkages.
  • Coordinates care with the treatment team.
  • Completes all tracking and reporting requirements for outcomes and evaluations.
  • Maintains appropriate professional standards and provides follow-up for clients as indicated.

Qualifications
  • Bachelors Degree in Health and Human Services
  • QHP or QHP eligible
  • 1 year of experience required
  • Ability to adapt service provision to incorporate all ages and diagnosis
  • Ability to coordinate care among multiple providers
  • Awareness of local resources and services
  • Excellent time management skills with proven ability to meet deadlines
  • Proficient with Microsoft Office Suite or related software
