Observation: this painting might be showing how he felt. Dealing with many life problems leading to his death, symbolized by the skull. His mental struggles leading to the hinge he didn’t want to do, symbolized by the burning cigarette. 
Interesting Facts: many believe this painting was showing his struggle against poverty which led him to smoke. However, his nephew claims otherwise. My observation was his paintings were all around the end of 1880s. also It says As he lay on his death bed, having apparently shot himself, Vincent Van Gogh is said to have uttered the final words, "The sadness will last forever"
