         <description><![CDATA[<div>There used to be only one psychiatric hospital in Belize and services were all centralised. This meant patients had a long way to travel for all outpatient appointments and follow-up.<br>Now there are 2 Psychiatric Nurse Practitioners based in each of the 8 district hospitals of Belize. They provide in- and out-patient treatment and do some home visits. They also assist other Primary Health Care professionals with managing mental disorders. <br>The service has performed well in evaluations but still has some logistical problems re: transport and long hours worked by the PNPs. </div>]]></description>

         <description><![CDATA[<div>In the Macul District of San Diago, general physicians diagnose mental illness and prescribe medications, however, they also refer for individual, group and family interventions run by psychologists. Family health team members provide supportive functions. Most of this takes place in mental health community centers where ongoing support and supervision for general physicians is also provided<br>. This system provides clear treatment pathways for service users and evaluation has shown that over time there has been an increase in the number of people identified and treated. </div>]]></description>

         <description><![CDATA[<div>MH added to Uganda Minimum Health Care Package and primary care workers trained, with out reach from specialist centres, aimed at improving access and improve outcomes whilst reducing disruption to client's lives. Poor data in general to support needs analysis (but extrapolation from local data). MH support integrated into the tiered health centre model. The integration of MH has had strong central support and local sensitisation (including of political leaders). 600 workers trained in MH and many others in other clinical workforce can prescribe most psychotropic medication from the essential drugs list.  Roles for primary care workers are: identify mental health problems, treat patients with uncomplicated<br>common mental disorders or stable chronic mental disorders, manage emergencies,<br>and refer patients who require changes in medication or hospitalisation.          <description><![CDATA[<div>Two participating primary care practices in a deprived area of London adapted their service to provide holistic, integrated care to vulnerable groups (eg. asylum seekers, refugees, homeless people) alongside their usual patient population. Their stepped approach involved informational materials and direction to useful resources, mental and psychosocial assessments leading to in practice management or referral as appropriate, referral to other organisations to assist with economic or social problems and ongoing integrated management of those with established mental illness, but who are stable. The practices also promoted good mental health and cultural awareness through additional training of staff.  Through these, and other measures, a large number of disenfranchised patients were managed effectively in primary care, providing a cost effective model for others to follow. </div>]]></description>

         <description><![CDATA[<div>Significant health reforms in Brazil over the past 30 years have seen a move away from institutional care in to community settings for the assessment and treatment of mental health difficulties.&nbsp; The community based mental health services are called CAPS (centres for psychological care) and consist of outpatient services, day treatment and partial hospitalisation services.&nbsp; There are now approximately 1000 CAPS across Brazil.&nbsp; CAPS are supposed to liaise with other health care services including primary care teams and hospitals, however the degree to which mental health is currently part of primary care can vary between municipalities.&nbsp; The model of collaborative care has become increasingly popular, with an example in the city of Sobral being held in high regard.&nbsp; Within this city, and through a evolved process of integration, mental health professionals work collaboratively with primary care health services to determine best treatment for individuals with mental health difficulties.&nbsp; Training and supervision also helps to strengthen the primary care team’s response to mental health. &nbsp;<br><br></div>]]></description>
