Children/young people should leave every placement with photo’s and memories. 

The journey should tell the story of how the child/young person came to live with their current carers and provides information about their birth family and reasons for decisions as well as hopes and dreams for the future.  

It should include timelines for decisions and significant events.  This book will become the child/young person possession and should always be written in a non-judgemental manner.  

Undertaking this work as less of an event, but something that progresses to be more in depth in time. The explanation can move with age and therefore a dynamic process that can continually be built on.

Can aid the expression of wishes and feelings and help children understand and articulate feelings; 
 
Help worker understand who is important to the child.  
 
The gathering of information and the discussion ensures that the child/ young persons journey is an ongoing and continuous process, which will be built on throughout child/ young persons time in care.

There are different professionals working with children who are looked after, who can all contribute to the making of this life journey book, at different times of the journey. 

Information should be included from:
- Birth Family  
- Foster family
- Nursery or Teachers
- Health professionals which include midwife/nurse/health  
   visitor
- Independent Reviewing Officer
- Court Guardian

These professionals will be provide information of the memories they have.  eg things the child/young person said/did.
