The second day of my internship at the Pusat Perubatan Qalam Laduni, I have learned how to key in patient's data in system. Normally, the patients made an appointment what time they want to be treated and what kind of treatment.

For those patients that have not been treated at the PPQL, the registration must be taken. So that the next treatment will be easier to find the patient's data if the data have been record in the systems.

The first thing that I have been learned at the PPQL, the communication between the patients is the most important things so that they will feel comfortable to communicate. The first thing that we must do is smile and welcoming the patients as soon as they enter the front door. I have learned the proper way how to record the patients data.

After the data have been record, the SN will assist the patients to check their blood pressure and then do PU (Pemeriksaan Pakar). After that, they will be suggest to do treatment such as Bekam lintah or any other treatment according to their illness.

Today my friend and I have been introduced about leech by the CEO of Qalam Laduni, Mr. Fahmi that have been used to treat patients. the scientific name of leech is Hirudo medicinalis. It is historically used in medicine to remove impure blood from patients. Leeches has both female and male reproductive organs. They use clitellum to hold their eggs and secrete the cocoon. Usually they keep the leeches in a big container that have been drilled holes and put oxygen pipe to keep them breathe. To ensure healthy leeches, water will be change everyday. Usually Qalam Laduni buy the leeches from the supplier with big a quantity which is 300 of them every three weeks.

Before the patients get the treatment of the leech, Ustaz will search the point at the foot and detect the pain experienced by patients. The effects of the treatment is the patients will feel itchy at the point where the leech bite. Normally, the structure of leech that have red color at their body is not suitable to be used as medicine. The right structure of leech is brown color at the front and have yellow line at their both side. In most blood-sucking leeches the digestive system starts with the jaws, three blades set at an angle to each other. In feeding they slice their way through the skin of the host, leaving a Y-shaped incision. Normally the leeches suck the patients blood in 2 hours or more. Then, the therapist will disposed the leeches that have been used by put it in a big amount of salt so that it will not suck other person's blood to avoid infection

Sharp at 9am, I have been commanded by Ustaz to assist him at the ward so that I can learn how he treat the patients. From my point of view, firstly, the most important thing is to know the patient's blood pressure. Then Ustaz will pinch each point of the patients body. It is used as traditional complementary medicine (TCM) which you only knew the illness without do any x-rays.

Today, I have the same shift with staff nurse, Ema but she need to run to the counter to assist Ustaz. So, it means that I have to take Ema's responsible in ward. Every activities that have been done by the patients such as Therapy, Reflexology, and Physiotherapy were recorded into the systems.

Then, at 12pm, Ema feeding the four patients that using Ryle tube feeding which are Sazili, Mazelan, Salbiah, and Sy. Zainab. I am not brave to do the feeding because I afraid that it will become worst such as abdominal bloating. The rights procedure are needed in feeding the patients. in lunch hour, as usual if there is not enough therapist, I fed Uncle Ahmad because he can't move his hand.

The new things that I have been learn today is to check patients BP. The machine called BP monitor device. The proper way to measure blood pressure using the machine is the patients must sit with good back support at the table or desk. But in the ward, all the patients are diagnosed with stroke can't sit properly. half of them only can lie down on the bed. Place the cuff on the patient's bare upper arm one inch above the bend of the patient's elbow. Make sure the tubing falls over the front center of the arm so that the sensor is correctly placed. Pull the end of the cuff so that it's evenly tight around the patient's arm. It should placed tight enough so that it only can slip two fingertips under the top edge of the cuff. Then press the start button and it will automatically measure the patient's blood pressure. It needs to measure in three times to get the accurate blood pressure.

Maybe they going back home to their hometown according to public holiday last Thursday.

I was sleepy back then and I don't know what to do at the counter. Mostly I only look at the television or scroll down my phone. But yesterday I've been learn one thing. The patients did the Terapi Alami at the back of Qalam Laduni bulding then, he ask me to check his Blood Pressure. He's having a raised systolic blood pressure but normal diastolic blood pressure is called Isolated Systolic Hypertension (ISH) and carries an increased risk of developing heart attacks or strokes and should be treated. Maybe his body moved out of his capability. from this case, I should have ask the patient to calm down and relax before check his blood pressure. End of my report today. thank you

Forgive me for my late update since I was sleepy yesterday and also my evening shift finished at 11pm. I was assigned to sit in the ward to take care of the patients. This is my first time work in the evening shift since the first day of intern at PPQL. The procedure just the same as at the morning shift but there were only two or three things different. First of all, at evening shift there is not have any physiotherapy activities and the staff nurse on duty, Kak Fatin gave the patients their medicine such as Aspirin, Insulin, Paracetamol, Bromexine, Epilim Syrup and many more.

As long as I enter the ward, Ustaz came to check on all the patients whether they had any improvement. This is my opportunity to observed Ustaz how he handle his patients. From my observation, Ustaz pinch all part of patient's body and search the important points. As I know, from the point, he can knew which is the right anatomy. Maybe I could learn from Ustaz so that I can treat my family one day InsyaAllah. Thats all from me today. Wait for my next update tonight. Adioss.

So now I'm gonna tell u about my routine yesterday which is not really different from the other day. Yesterday I was in charge at the registration counter and luckily from 6pm until 8pm there was no booking session with the Ustaz. So, in order to wait until 9pm, I was wondering around at the counter and sometimes I played with the staff's children. Haha. So that I don't feel lonely at the counter.

And then sharp at 9pm the patient slowly come to the PPQL to get treated. So, I left my phone or whatever behind whether it ringing. But I don't care because I'm on duty that day. I'm a little bit sleepy right now plus I'm too hungry eventhough I had my lunch today at 3pm with my partner, Tini. Today, I able to assist staff nurse Nurul in the ward together with Tini. Luckily, I able to learn how to measure SPO2 among the patients. The purpose of SPO2 is to measure how much oxygen in your body. SPO2 stands for peripheral capillary oxygen saturation, an estimate of the amount of oxygen in the blood. more specifically, it is the percentage of oxygenated haemoglobin (haemoglobin containing oxygen) compared to the total amount of haemoglobin in the blood (oxygenated and non-oxygenated haemoglobin). Then, after measure all the patients, I and Tini key in all the patient's data in the systems. Down here I would like to attach together with the SPO2 machine for you to know.

Yesterday was my evening shift and I don't have enough time to update my padlet since my shift finished at 11pm. I was tired back then and now I'm free to update my padlet for today and yesterday. Yesterday I was assigned to duty in the ward. Since all the patients in the ward were diagnosed with stroke, I was wondering how the stroke attack the patients. So now I will tell you all about stroke.

A stroke sometimes called a "brain attack," occurs when blood flow to an area in the brain is cut off. The brain cells, deprived of the oxygen and glucose needed to survive, die. If a stroke is not caught early, permanent brain damage or death can result.

How does the stroke occurs?

Ischemic stroke is similar to a heart attack, except it occurs in the blood vessels of the brain. Clots can form in the brain's blood vessels, in blood vessels leading to the brain, or even in blood vessels elsewhere in the body and then travel to the brain. These clots block blood flow to the brain's cells. Ischemic stroke can also occur when too much plaque clogs the brain's blood vessels. About 80% of all strokes are ischemic.

Hemorrhagic strokes occur when a blood vessel in the brain breaks or ruptures. The result is blood seeping into the brain tissue, causing damage to brain cells. The most common causes of hemorrhagic stroke are high blood pressure and brain aneurysms. An aneurysms is a weaknesses or thinness in the blood vessel wall.

What Are the Symptoms of Stroke?

Weakness or numbness of the face, arm, or leg on one side of the body

Loss of vision or dimming (like a curtain falling) in one or both eyes

Loss of speech, difficulty talking, or understanding what others are saying

Sudden, severe headache with no known cause

Loss of balance or unstable walking, usually combined with another symptom

After that incident I realize that we can't satisfied all our customer. Different people come with different taste. Some customer would like to request who is gonna treat them. So to fulfill their needs, we have to implement their request. So I would like to say that I have to be patient until 26th May 2017.

Sorry for the late update. Today, I want to talk about Bellpalsy. There are a few of patients who came to the PPQL diagnosed with this disease.

What is Bell's palsy?

Bell's palsy is a paralysis or weaknesses of the muscles on one side of your face. Damage to the facial nerve that controls muscles on one side of the face causes that side of your face to drop. The nerve damage may also affect your sense of taste and how you make tears and saliva. This condition comes on suddenly, often overnight, and usually gets better on its own within a few weeks.

Bell's palsy is not the result of a stroke or a transient ischemic attack (TIA). While stroke and TIA can cause facial paralysis, there is no link between Bell's palsy and either of these conditions. But sudden weakness that occurs on one side of your face should be checked by a doctor right away to rule out these more serious causes.

What causes Bell's palsy?

The cause of Bell's palsy is not clear. Most cases are thought to be caused by the herpes virus that causes cold sores.

In most cases of Bell's palsy, the nerve that controls muscles on one side of the face is damaged by inflammation.

Many health problems can cause weakness or paralysis of the face. If a specific reason cannot be found for the weakness, the condition is called Bell's palsy.

What are the symptoms?

Symptoms of Bell's palsy include:

Sudden weakness or paralysis on one side of your face that causes it to drop. This is the main symptom. It may make it hard for you to close your eye on that side of your face.

Drooling.

Eye problem, such as excessive tearing or a dry eye.

Loss of ability to taste.

Pain in or behind your ear.

Numbness in the affected side of your face.

Increased sensitivity to sound.

How is Bell's palsy diagnosed?

Your doctor may diagnose Bell's palsy by asking you questions, such as about how your symptoms developed. He or she will also give you a physical and neurological exam to check facial nerve function.

If the cause of your symptoms is not clear, you may need other tests, such as blood tests, an MRI, or a CT scan.

How is it treated?

Most people who have Bell's palsy recover completely, without treatment, in 1 to 2 months.1 This is especially true for people who can still partly move their facial muscles. But a small number of people may have permanent muscle weakness or other problems on the affected side of the face.

Treatment with corticosteroid medicines an make it more likely that you will regain all facial movement. They work best if they are taken soon after symptoms start (within 3 days). Sometimes antiviral medicines may be added to corticosteroid medicines to treat Bell's palsy. But evidence for using antiviral medicines is weak. They may help in some cases, but in general they do not affect recovery.
</sup>

Some people may not be able to take corticosteroid medicines because of other health problems. It's important to remember that most people with Bell's palsy recover completely without any treatment.

How can you care for yourself at home?

Facial exercises. As the nerve in your face begins to work again, doing simple exercises-such as tightening and relaxing your facial muscles-may make those muscles stronger and help you recover more quickly. Massaging your forehead, cheeks, and lips with oil or cream may also help.

Eye care. If you can't blink or close your eye fully, your eye may become dry. A dry eye can lead to sores and serious vision problems. To help protect the eye and keep it moist:

Use your finger to close and open your eyelid often throughout the day.

Use eyedrops or ointment. Those that contain methylcellulose are a good choice and don't require a prescription. You may want to use drops during the day and ointment at night while you sleep. Ask your doctor how often to use the drops.

Wear an eye patch while you sleep, and wear glasses or goggles the rest of the time.

Mouth care. If you have no feeling and little saliva on one side of your tongue, food may get stuck there, leading to gum disease or tooth decay. Brush and floss your teeth often and well to help prevent these problems. To prevent swallowing problems, eat slowly and chew your food well. Eating soft, smooth foods, such as yogurt, may also help.

Sorry again for the late update of my padlet. Yesterday I was duty at the registration counter and I felt very bad due to stomachache. But I still stay strong to work. Many of patient who came to PPQL have different type of disease. But there is one disease that I have been interested. It's called Epilepsy. The patient was in 6 years and he's a boy. So now I will talk all about Epilepsy.

What is Epilepsy?

Epilepsy is a condition in which a person has recurrent seizures. A seizure is defined as an abnormal, disorderly discharging of the brain's nerve cells, resulting in a temporary disturbance of motor, sensory, or mental function.

There are many types of seizures, depending primarily on what part of the brain is involved. The term epilepsy says nothing about the type of seizure or cause of the seizure, only that the seizures happen again and again. A stricter definition of the term requires that the seizures have no known underlying cause. This may also be called primary or idiopathic epilepsy.

Episodes of abnormal electrical activity within the brain result in seizures.

The specific area of the brain affected by the abnormal electrical activity may result in a particular type of seizure.

If all areas of the brain are affected by the abnormal electrical activity, a generalized seizure may result. This means that consciousness is lost or impaired. Often all the person's arms and legs stiffen and then jerk rhythmically.

One seizure type may evolve into another during the course of the seizure. For example, a seizure may start as a partial, or focal, seizure, involving the face or arm. Then the muscular activity spreads to other areas of the body. In this way, the seizure becomes generalized.

Seizures caused by high fevers in children are not considered epilepsy. Also see children's seizures.

Epilepsy Causes

Healthy people may have seizures under certain circumstances. If the seizures have a known cause, the condition is referred to as secondary or symptomatic epilepsy. Some of the more common causes include the following:

Chemical imbalance such as low blood sugar or sodium

Head injuries

Certain toxic chemicals or drugs of abuse

Alcohol withdrawal

Stroke, including hemorrhage

Birth injuries

Tumor

Epilepsy Symptoms

Almost any type of behavior that happens repetitively may represent a seizure.

Generalized seizures: All areas of the brain are involved in a generalized seizure. Sometimes these are referred to as grand mal seizures.

To the observer, the person experiencing such a seizure may cry out or make some sound, stiffen for some seconds, then have rhythmic movements of the arms and legs. Often the rhythmic movements slow before stopping.

Eyes are generally open.

The person may not appear to be breathing. The person is often breathing deeply after an episode.

The return to consciousness is gradual and should occur within a few moments.

Loss of urine is common.

Often people will be confused briefly after a generalized seizure.

Partial or focal seizures: Only part of the brain is involved, so only part of the body is affected. Depending on the part of the brain having abnormal electrical activity, symptoms may vary.

If the part of the brain controlling movement of the hand is involved, for example, then perhaps only the hand may show rhythmic movements or jerking.

If other areas of the brain are involved, symptoms might include strange sensations or small repetitive movements such as picking at clothes or lip smacking.

Sometimes the person with a partial seizure appears dazed or confused. This may represent a partial complex seizure. The term "complex" is used by doctors to describe a person who is between being fully alert and unconscious.

Absence or petit mal seizures: These are most common in childhood.

Impairment of consciousness is present with the person often staring blankly.

Repetitive blinking or other small movements may be present.

Typically, these seizures are brief, lasting only seconds. Some people may have many of these in a day.

Other seizure types exist particularly in very small children.

Today I was duty at the registration counter. There were not many customer today due to the "solat Jumaat". But we still got customer to handle. So now, I want to talk about High Blood Pressure (Hypertension).

Definition

High blood pressure is a common condition in which the long-term force of the blood against your artery walls is high enough that it may eventually cause health problems, such as heart disease.

Blood pressure is determined both by the amount of blood your heart pumps and the amount of resistance to blood flow in your arteries. The more blood your heart pumps and the narrower your arteries, the higher your blood pressure.

You can have high blood pressure (hypertension) for years without any symptoms. Even without symptoms, damage to blood vessels and your heart continues and can be detected. Uncontrolled high blood pressure increases your risk of serious health problems, including heart attack and stroke.

High blood pressure generally develops over many years, and it affects nearly everyone eventually. Fortunately, high blood pressure can be easily detected. And once you know you have high blood pressure, you can work with your doctor to control it.

Symptoms

Most people with high blood pressure have no signs or symptoms, even if blood pressure readings reach dangerously high levels.

A few people with high blood pressure may have headaches, shortness of breath or nosebleeds, but these signs and symptoms aren't specific and usually don't occur until high blood pressure has reached a severe or life-threatening stage.

Causes

There are two types of high blood pressure.

1. Primary (essential) hypertension

For most adults, there's no identifiable cause of high blood pressure.

Today I was duty in the ward and as usual my task is to record all the activities that have been made by the patients. But today, I want to talk about Hypotension since every time I checked all the patient's BP, they always ask what is the normal range for normal blood pressure that we could have.

Hypotension

Low blood pressure also known as hypotension where blood pressure in your arteries is abnormally low. Naturally, low blood pressure is unlikely to cause any symptoms and is normally nothing to worry about. But if your blood pressure drops too low, it can restrict the amount of blood flowing to your brain and other vital organs, which can cause unsteadiness, dizziness or fainting.

What is low blood pressure?

The heart pumps a constant supply of blood around the body through arteries, veins and capillaries. Blood pressure is a measure of the force of the blood on the walls of the arteries as the blood flows through them.

It's measured in millimetres of mercury (mmHg) and recorded as two measurements:

systolic pressure – the pressure when your heart beats and squeezes blood into your arteries

diastolic pressure – the pressure when your heart rests between beats

For example, if your systolic blood pressure is 120mmHg and your diastolic blood pressure is 80mmHg, your blood pressure is 120 over 80, which is commonly written as 120/80.

Normal blood pressure is between 90/60 and 140/90. If you have a reading of 140/90 or more, you have high blood pressure (hypertension). This puts you at greater risk of developing serious health conditions, such as heart attack or stroke.

People with a blood pressure reading under 90/60 are usually regarded as having low blood pressure.

Why do I have low blood pressure?

You can have low blood pressure for many reasons, including the time of day, your age, the temperature, any medication you may be on, an injury, and some illnesses.

Treatment

Naturally low blood pressure doesn't usually need to be treated unless it's causing symptoms such as dizziness or recurrent falls. There are also various things you can do to help limit symptoms of low blood pressure, including:

standing up gradually and avoiding standing for long periods of time

ensuring you are well hydrated

wearing support stockings

adding more salt to your diet eating smaller meals more often

So now let me introduce a little bit about Ryle tube feeding. Ryle tube feeding also known as nasogastric tube (NG tube) which is a special tube that carries food and medicine to the stomach through the nose. It can be used for all feedings or for giving a person extra calories. Good nutrition is needed for your body to heal. Fluid is needed to prevent your body from getting dried out (dehydrated).

Take a good care of feeding bag and tubing is very important to make sure that they work properly. To avoid the skin around the nostrils, get irritated, a good care of the skin around it is very important.

Flushing the tube will help release any formula stuck to the inside of the tube. Flush the tube after each feeding. Other than that, there are important step to feed the patient to avoid from getting choking or difficulty breathing while feeding. Before or after feeding, fill the food container with the amount of water that the doctor prescribes. This provides fluids for you and flushes out the tube.

Then, while waiting for their bath, other ‘perawat’ change their bed sheets. After that, at 7am all the patients will take their breakfast. Then, from 8am until 12pm, the ‘perawat’ will conduct the patients in doing massage and physiotherapy watched by the staff nurse. Sharp at 12pm, the patients have their lunch. I forgot to mention that the diaper must change every 4 hours before they eat and before they sleep. At evening also ‘perawat’ must do reflexology to all the patients. At 6.30pm the patients will have their dinner. It is always given rice, fried fish, and vegetables with plain water. Here in PPQL, chicken is not allowed to give all the patients. At night, sharp at 10pm the staff nurse will switch off the lamp to let the patients sleep. Same goes to everyday.

Slip disc

Ok now let me talk about slip disc since there are too many patients who came to get treated at PPQL have the same problem with their back pain. Slip disc occurs when the soft inner portion of the disk protrudes through the outer ring. Symptoms vary based on where the slipped disk occurs and if it compresses any of your spinal nerves. Slipped disk are not only common in older adults but also teenagers.

The spinal column is made up of a series of bones stacked onto each other. From top to bottom, the column includes seven bones in the cervical spine, 12 in the thoracic spine, and 5 in the lumbar spine, followed by the sacrum and the coccyx at the base. These bones are cushioned by disks. The disks protect the bones by absorbing the shocks from daily activities like walking, lifting, and twisting.

The lower back is one of the more common areas for slipped disks. Your spinal column is an intricate network of nerves and blood vessels. A slipped disk can place extra pressure on the nerves and muscles around it.

Symptoms are:

- Pain and numbness, most commonly on one side of the body

- Pain that extends to your arms or legs

- Pain that worsens at night or with certain movements

- Pain that worsens after standing or sitting

- Pain when walking short distances

- Unexplained muscle weakness

- Tingling, aching, or burning sensations in the affected area

How to treat?

Usually at PPQL, Ustaz will suggest the patients to do ‘bekam lintah’ at the certain point that he/she feel pain. The highlight of the treatment is to remove the dirty blood or blood cloth at the certain points. Usually, after the treatment, the patients said that their body feel energized and more fresh. In medical method maybe it totally different but here I’m just want to talk about traditional medicine complementary that PPQL have been used to offer to the customer.

Diabetes

Intro

Half of the patients in the ward have been diagnosed with diabetes. Before that, we have a vote among us and I got Belalai Gajah which is also known as Sabah Snake Grass. Let me tell you about it since I also don’t know before.

Sabah Snake Grass is an herb grown usually in tropical weather countries, like Malaysia and Thailand. Its scientific name is Clinacanthus nuthans. It is a non-toxic edible plant. In Thailand they used it for treatment of skin rashes, and herpes, and also used to cure snake bites. Besides that, it is also used to treat patients with kidney failure in Indonesia. While in Malaysia, it is famous for having good effects on cancer patients, although it is also used to reduce symptoms of dialysis patients.

Research shows that Clinicanthus Nutans is a kind of edible plant, ant it contains extremely high levels of Flavonoids. It is containing a multiple of vitamins, minerals and amino acids that the body requires. Flavonoids is a substance that has anti-inflammatory effects and anti-oxidative effects. Rutin and Quercetin are the most important type Flavonoids. Rutin is routinely used for the treatment of gum inflammation and shows good effects. On the other hand, Quercetin can inhibit the negative effects of carcinogens. Soy bean is known to have large about of Flavonoids and thus soy bean milk is good for health.

I feel so sleepy because last night I watched Japanese drama. Hehehe Sorry for that. Ok now I want to tell you about the benefits of the other herbs that we had planted. In Malay, we call it as ‘sambung nyawa’ but in English we call it as Longevity Spinach. But it’s scientific name is Gynura procumbens. It is found in China, Southeast Asia, and Africa. The plant grows wild but is also cultivated as a vegetable or medicinal plant. Its young leaves are used for cooking, such as with meat and prawns in a vegetable soup.

It has found its way around the world and is widely used as an accepted medicine for diabetes, HTN, and elevated levels of cholesterol and triglycerides. In Singapore, Gynura procumbens is believed to lower blood pressure and sugar levels. Locals there will recommend you to eat 3 – 7 leaves daily in the raw form. It also has been used for the treatment of eruptive fevers, rash and kidney disease. The leaves of this plant continue to be used as folk medicine to control diabetes mellitus and hyperlipidemia. Besides, it also protects the kidneys and retinas from damage caused by high blood sugar.

Today is my off day but we had class with Ustaz. An hour class have been talk about the philosophy of Malay Traditional Medicine. It started with post-natal during the prophet Adam a.s when Siti Hawa giving birth. Our body have 4 elements which is fire (heat), earth (bones and muscles, water (blood), and air.

Imbalance of Elements and Disease:

1) Imbalance of Water element

- Cold, sinusitis, asthma, swellings, blood thinning or blood clotting, problem of urination, diseases of reproductive organs.

2) Imbalance of Earth element

- Weakness, obesity, cholesterol, weight loss, and weight gain, bones or muscular diseases.

3) Imbalance of Fire element

- Coldness or fever, skin diseases, increased coldness or heat in body, loss of vital energy, acidity, diabetes, and mental disorders.

4) Imbalance of Air element

- Nervous disorders, blood pressure problems, lung disorders, physical pains/deformities, depression, locomotors ataxia.

Before the modern medicine exists, the way they treat patient is based on 3 aspects which are time, day, and month. By these 3 aspects they can diagnosed the pain of the patient. Other than that, they also used the birth date as their guide. Besides, they also diagnosed through the patient’s name which is calculate the number of letters and associated with the 4 elements of human body.

Previous area, they measure temperature by using betel leaves or coleus blumei leaves. The leaves then put in the armpits and if the leaves look bruise, it means that the temperature of your body is high. To measure air of your body is by using wet cotton and put it between your nose and mouth. After a few minutes, if the cotton dry it means that air in your body is hot. Then lastly, to measure blood is to check your pulse at the carotid artery (neck). If your pulse is faster, it means that your body is hot.

Today I want to explain the function of DXT tools. DEX stands for (DXT) Blood Sugar which is the tools to measure blood sugar in your body. They all involve pricking a finger with a needle to get a drop of blood, applying the blood to a testing strip, and having the monitoring machine measure the glucose from the testing strip. There are various “pokers” (lancing devices) on the market. Some are less painful than others, by virtue of an adjustable depth of penetration of the needle (lancet), the fitness of the needle, how much vibration is produced, and so on. The specific type of monitor and lancing device appropriate for you should be discussed with your diabetes educator.

The reasons why must check your blood sugar.

Monitoring shows you what your blood glucose is doing at the best of times, while you are feeling good and in your usual routine. This allows you to interpret the readings under more unusual circumstances. It also lets you to catch any changes in your glucose as time goes by.

It shows you how your blood glucose varies over the course of the day, indicating trends.

It shows you what happens to your blood glucose if there is a change in your diet, such as going out to supper.

It shows you what happens to your blood glucose when there is a change in physical activity, such as playing sports.

It shows you what is happening to your blood glucose in times of illness.

It shows you what happens to your glucose when there is a change in medication (for diabetes or any other illness), and where further changes need to be made.

To let you know if a sensation that you are feeling is due to a glucose that is too low or too high.

If recorded properly, it shows your doctor all of the above so that he can better advise you about your diabetes medications.

Blood glucose levels are measured in millimoles per litre of blood (mmol/L). Target ranges may differ depending on your age, duration of diabetes, the type of medication you are taking and if you have any other medical problems. Normal blood glucose levels are between 4.0–7.8mmol/L. If you level above 7.8mmol/L, the risk to have a diabetes is high.

Spirit is the nonphysical part of a person that is the seat of emotions and character which is soul. Besides, spirit is also known as those qualities regarded as forming the definitive or typical elements in the character of a person, ration, or group or in the thought and attitudes of a particular period.

For muslim, spirit or we call it as “roh” have been completed the creation of human, it can occur anytime and also as a practitioner, you can analyze the type of illness of the patients by look at the patient’s reaction, character and attitudes. For example, Balpalsy include (rohani + jasad) while Cikgu Ezzu (rohani) due to anxiety disorder. That’s all for today. Thank you for giving your time. :P

The swelling can occur in one particular part of the body or may be more general, depending on the cause. Fluid can accumulate under the skin – most commonly causing swelling of the lower legs and ankles – or in one of the body's internal organs, such as the lungs. As well as swelling or puffiness of the skin, oedema can also cause:

skin discolouration

areas of skin that temporarily hold the imprint of your finger when pressed (known as pitting oedema)

aching, tender limbs

stiff joints

weight gain

Types of oedema

Oedema is most common in the feet and ankles.

Today I want to share with you guys about the time of visit to the ward of Qalam Laduni Medical Centre. Most hospital have times at which you can visit your friend or relative. For proper visit, please check with the relevant hospital or any kind of medical centre for information about when you can visit. Bear in mind that not all the time the patient is free because they need to do some exercising and some therapy.

If you are unable to attend during visiting hours, talk to the member of staff in charge of the ward to arrange an alternative time to visit. Hospitals encourage relatives and friends to visit patients in order to give them moral support. However, patients can get tired very quickly. For this reason, the number of visitors each patient is allowed is usually restricted, and it might be necessary to stagger the visitors so they come at different times.

From my observation in time of visiting in PPQL, sometimes the visitor does not obey the instruction of the time visit. There is timetable that attached at the front door of the ward the perfect timing for visit the patients. They still visit the patients while they are under treatment. So, it makes our work delayed. My suggestion is to strengthened the rule in the ward.

If you’re a productive employee with a good attitude who exhibits professional behavior, you can help the business to succeed and further your career.

Today I want to share with you guys about employers’ right. Employers are responsible for ensuring all their employees receive certain basic employment rights. These rights are governed by detailed employment legislation. If you employ people or are setting up a business that will employ people you need to be familiar with your responsibilities and your employees’ rights.

Definition of an employee

Employers engage persons on either contracts of service or contracts for services. Only a person engaged under a contract of service is an employee and therefore protected by the full range of employment legislation. An independent contractor or self-employed person will have a contract for services with the party for whom the work is being done. The distinction between a contract of service, and a contract for services can sometimes be unclear but the type of contract a person is engaged under can have serious implications for both employer and employee in matters such as employment protection legislation, taxation and social welfare.

Contracts and terms of employment

While the full contract of employment does not have to be in writing, you must give your employee certain terms and conditions of employment in writing within 2 months of starting employment. This information must include the full names of the employer and the employee and details of the job title, the pay, hours of work, notice requirements. You can read details in our document on the contract of employment.

While most terms and conditions of employment are stated in a written contract or a company handbook you should be aware that custom and practice in the workplace can also constitute a term of employment, for example, a mid-morning break of 10 minutes.

If you, as the employer, wish to change a term or condition of employment you must agree this change with your employee. This requirement for both the employer’s and the employee’s consent to changes in the terms of the contract is part of contract law. Based on PPQL, they do not use this system towards their employees. So if anything happens to the employees, they can’t summon the company. So this is their weakness.

Hours of work, breaks and rest periods

You are responsible for ensuring that your employees are given adequate rest. The Organisation of Working Time Act 1997 sets down the rules governing maximum working hours and daily and weekly rest breaks. Based on my observation, the “perawat” does not have enough time to have a rest even to eat especially at weekend. They usually eat and rest or call their family at the time where there is no customer. Their time working s from 4am till 12am nonstop. That is why sometimes they look very tired.

Leave

Nearly all employees, full-time, part-time, temporary or casual have annual leave and public holiday entitlements from the time they start work. Most employees are entitled to 4 weeks’ paid annual leave per leave year. Part-time workers ‘entitlement is generally calculated as 8% of the hours worked subject to a maximum of 4 working weeks per leave year. Employers can determine the timing of annual leave, taking into consideration work and personal requirements; however, you should consult your employee or their union in advance. Your employee can request pay for annual leave in advance. You are also obliged to allow employees to avail of statutory protective leave, such as maternity leave, paternity leave, health and safety leave, parental leave, adoptive leave, and care’s leave. There is specific legislation setting down the rules for each entitlement.

Based on my observation, the staff nurse only had one off day in a week. To apply for off day need to have a valid reason. So my suggestion is if you want to have a dedicated and a great performance from your employees, give them reward or holiday package to motivate themselves to work harder in order to achieve organization’s vision and mission.

We love hiking so much because it is challenging actually. It includes the physical and mental endurance of the person. To have the “taiko” of the hiking in our group who were Leen and Abby, it motivates our self to hike until the hilltop. Our duration to achieve at the hilltop is about an hour. Finally, we can see the sunrise and Subhanallah it so beautiful. I can’t describe how beautiful it is to look from the top hill. From that side, we can see full of buildings. After that, we return back home and I start my shift at the evening because I feel so tired. Then, I start my shift as usual. Thank you Cik Fahmi. J

Today I want to share with you guys about this disease which is gout. There are many patients with this disease come to PPQL to get a treatment. Gout is a type of arthritis in which small crystals form inside and around the joints. It causes sudden attacks of severe pain and swelling.

The condition mainly affects men over 30 and women after the menopause. Overall, gout is more common in men than women. Gout can be extremely painful and debilitating, but treatments are available to help relieve the symptoms and prevent further attacks. Any joint can be affected by gout, but it usually affects joints towards the ends of the limbs, such as the toes, ankles, knees and fingers.

Signs and symptoms of gout include:

severe pain in one or more joints

the joint feeling hot and very tender

swelling in and around the affected joint

red, shiny skin over the affected joint

Symptoms develop rapidly over a few hours and typically last three to 10 days. After this time the pain should pass and the joint should return to normal. Almost everyone with gout will experience further attacks at some point, usually within a year.

What causes gout?

Gout is caused by a build-up of a substance called uric acid in the blood. If you produce too much uric acid or your kidneys don't filter enough out, it can build up and cause tiny sharp crystals to form in and around joints. These crystals can cause the joint to become inflamed (red and swollen) and painful.

Things that may increase your chances of getting gout include:

obesity, high blood pressure and/or diabetes

having a close relative with gout

kidney problems

eating foods that cause a build-up of uric acid, such as red meat, offal and seafood

drinking too much beer or spirits

Treatments for gout

If you have gout, treatment is available from your GP to:

relieve symptoms during an attack – this can be done using ice packs and by taking medications such as non-steroidal anti-inflammatory drugs (NSAIDs), colchicine or corticosteroids

prevent further attacks – through a combination of lifestyle changes, such as losing weight or changing your diet, and taking medication that lowers uric acid levels, such as allopurinol

Today I want to share with you guys about diabetes. Diabetes is a lifelong condition that causes a person's blood sugar level to become too high.

There are two main types of diabetes:

type 1 diabetes – where the body's immune system attacks and destroys the cells that produce insulin

type 2 diabetes – where the body doesn't produce enough insulin, or the body's cells don't react to insulin

Type 2 diabetes is far more common than type 1. During pregnancy, some women have such high levels of blood glucose that their body is unable to produce enough insulin to absorb it all. This is known as gestational diabetes.

Pre-diabetes

Many more people have blood sugar levels above the normal range, but not high enough to be diagnosed as having diabetes. This is sometimes known as pre-diabetes. If your blood sugar level is above the normal range, your risk of developing full-blown diabetes is increased. It's very important for diabetes to be diagnosed as early as possible because it will get progressively worse if left untreated.

When to see a doctor

feeling very thirsty

urinating more frequently than usual, particularly at night

feeling very tired

weight loss and loss of muscle bulk

itching around the penis or vagina, or frequent episodes of thrush

cuts or wounds that heal slowly

blurred vision

Type 1 diabetes can develop quickly over weeks or even days. Many people have type 2 diabetes for years without realizing because the early symptoms tend to be general.

Causes of diabetes

The amount of sugar in the blood is controlled by a hormone called insulin, which is produced by the pancreas (a gland behind the stomach). When food is digested and enters your bloodstream, insulin moves glucose out of the blood and into cells, where it's broken down to produce energy.

However, if you have diabetes, your body is unable to break down glucose into energy. This is because there's either not enough insulin to move the glucose, or the insulin produced doesn't work properly. Although there are no lifestyle changes you can make to lower your risk of type 1 diabetes, type 2 diabetes is often linked to being overweight.

What is eczema?
Common symptoms of eczema include areas of thickened, red, itchy skin. If both parents have an atopic disease, the chances increase further.

Environmental factors are also known to bring out the symptoms of eczema. These include:

Irritants - soaps, detergents, shampoos, disinfectants, juices from fresh fruits, meats, or vegetables

Allergens - dust mites, pets, pollens, mold, dandruff

Microbes - bacteria such as Staphylococcus aureus, viruses, certain fungi

Hot and cold temperatures - hot weather, high and low humidity, perspiration from exercise

Foods - dairy products, eggs, nuts and seeds, soy products, wheat

Stress - it is not a cause of eczema but can make symptoms worse

Hormones - women can experience worsening of eczema symptoms at times when their hormone levels are changing, for example during pregnancy and at certain points in their menstrual cycle

Symptoms of eczema

Atopic dermatitis commonly manifests itself in infants with dry and scaly patches appearing on the skin. These patches are often intensely itchy. The symptoms of atopic dermatitis can vary, depending on the age of the person with the condition.

Most people develop atopic dermatitis before the age of 5. Half of those who develop the condition in childhood continue to have symptoms as an adult, though these symptoms are often different to those experienced by children.

People with the condition will often experience periods of time where their symptoms will flare up or worsen, followed by periods of time where their symptoms will improve or clear up.

Infants:

Rashes commonly appear on scalp and cheeks.

Rashes usually bubble up before weeping fluid.

Rashes can cause extreme itchiness, which may lead to trouble sleeping. Continuous rubbing and scratching can lead to skin infections.

Children, from 2 years old to puberty:

Rashes commonly appear behind the creases of elbows or knees

Also common on neck, wrists, ankles, crease between buttock and legs

Over time, the following symptoms can manifest:

Rashes can become bumpy, like goosebumps

Rashes can lighten or darken in color

Rashes can thicken (also known as lichenification) and then develop knots and a permanent itch

Adults:

Rashes commonly appear in creases of elbows or knees or nape of neck

Rashes cover much of the body

Rashes can be especially prominent on neck, face, and around the eyes

Rashes can cause very dry skin

Rashes can be permanently itchy

Rashes can cause scaly skin (scalier than in children)

Rashes can lead to skin infections

Adults who developed atopic dermatitis as a child but no longer experience the condition may still have dry or easily irritated skin, hand eczema, and eye problems.

The appearance of skin affected by atopic dermatitis will depend on how much a person scratches and whether the skin is infected. Scratching and rubbing irritates the skin further, increases inflammation and makes itchiness worse.

Arteritis refers to inflammation of your arteries that damages your blood vessel walls and reduces blood flow to your organs. There are several types of arteritis. The symptoms and complications that occur depend on which arteries are affected and the degree of damage.

What Are the Types of Arteritis?

1) Takeyasu’s Arteritis

2) Giant Cell Arteritis (GCA)

3) Polyarteritis Nodosa (PN)

What Causes Arteritis?

No one knows what causes arteritis. It’s believed to be an autoimmune disorder. Your immune cells attack the walls of your major blood vessels, causing varying degrees of damage. The immune bodies inside your blood vessels form nodules called granulomas that block blood flow to other parts of your body. The interior cells of your blood vessels may be weakened, making them prone to aneurysms. Aneurysms can rupture and cause internal bleeding.

These diseases may lead to a loss of blood and oxygen to your internal organs. Impaired blood flow to your body organs can cause blindness, heart failure, or kidney failure, depending on which arteries are involved. Rapid treatment is necessary to prevent organ damage from arteritis.

How Is Arteritis Treated?

The treatment of arteritis focuses first on suppressing your immune reaction. To do this, your doctor will prescribe corticosteroids, such as prednisone, or other drugs that suppress your immune system. Your doctor can bring secondary infections under control using antibiotics. Finally, if any of your organs has been damaged, you may need surgery to restore them to a healthier state.

Today I want to explain about the insulin. Insulin is a natural hormone made by the pancreas that controls the level of the sugar glucose in the blood. Insulin permits cells to use glucose for energy. Cells cannot utilize glucose without insulin.

Insulin is a medicine to control the level of sugar in your body for those who diagnosed diabetes. The person with type 1 diabetes must inject exogenous insulin (insulin from sources outside the body).

In addition to promoting the access of glucose into cells, insulin is also called a builder hormone. This is because it helps fat and muscle to form. It promotes the storage of glucose in the form of glycogen for times when glucose is not coming in. It also blocks the breakdown of protein. The patients who are under insulin are Jeffree, Semiah and Rohimah. They are given insulin every day. For Rohimah, she has to inject 3 times a day and for Jeffree and Semiah only once a day at night.

Insulin Tips:

· Insulin can be kept at room temperature for 4 weeks or kept in a refrigerator until its expiry date.

There was a patient who diagnosed with cervical cancer and she came here to get another alternative treatment beside hospital. Cervical cancer occurs when abnormal cells on the cervix is the lower part of the uterus that opens into the vagina. Cervical cancer can often be successfully treated when it’s found early.

Symptoms

· Bleeding from the vagina that is not normal, such as bleeding between menstrual periods, after sex, or after menopause

· Pain in the lower belly or pelvis

· Pain during sex

· Vaginal discharge that isn’t normal

The patient said that she had a surgery to remove the cell cancer but afraid to do chemotherapy. Because she heard that many people who did the chemotherapy died after a few weeks. But for me, it is good to do the chemotherapy at least you try your best to get treated. Tawakal to Allah is the most important thing. But Ustaz said to the patient no need to get a chemotherapy and just get the treatment from him. For me, it was totally wrong to say that to the patient who is diagnosed with servical cancer. At least you have to give her spirit and motivate her to do the chemotherapy. Honestly, I am not agreed with Ustaz statement. Because my mother also diagnosed with breast cancer and she have been through chemotherapy twice. Alhamdullilah, now she is healthy and strong enough to do daily activities.

Unluckily, there was a patient who came with family and did the treatment together but there is not enough "perawat". But there was one patient who insist to do the "bekam lintah" at 11 pm. Usually, the treatment of "bekam lintah" take about 2 or 3 hours. I've explained to her but she still insist to do bekam. So to fulfill her request, I've to do it eventhough my shift end at 11pm. So today I am going back home at 11.30pm. I don't care because we must finish our task first before pass it to another.

Today at 6.30 am, me and my partner did an activity together with our SV. We had plan before to go hiking at Bukit Saga and had no preparation at all in terms of physical. We start to climb up the Bukit Saga at 8am without warm up and finally it's a tiring day for me. Hahahah 😂 The track is almost tough compare to Bukit Broga the other day. But it's not going to break my spirit to fight till to the top. Luckily, I have my friends who always there to help me. Thanks to Abby, Leen and Tini who always there eventhough they always teasing me. Heheheh. 😁 Along the journey to the uphill of the Bukit Saga, I heard the sound of Ungka and I really missed it because everytime I'm hiking, it always there make a noise. 🐒 The track is quite tough that makes me fall twice infront on my SV. But I'm cool and relax then it feels pain at my buttocks. Huhu 😭 After reach at the too of Bukit Saga, we then continue hiking to see the waterfall and it takes about 40 minutes. Subhanallah, it was very beautiful and we take about an hour to rest. It was fun actually if you do an activity sometimes instead of just go to the office doing the same work. We come back home at 2pm and had lunch before we go to work as usual at 4pm.

It also bears a white flower with purple spot on it.the unique characteristics about cekur is the leaf and roots have a distinctive aromatic smell.

Actually today we were assigned to work on morning shift and one of our teammates had to take sudden leave because she was not feeling well. So, we can't do anything because we already enter morning shift. For my opinion, if she tells earlier, we can cover it up for her. But now, we can't do double shift because it is out of our ability. So, we let the management settle the problem themselves. Sorry for not helping. Huhu.

Today is morning shift again with my partner, Leen. There was not much different task given but while assist Ustaz in the examination room, there was a patient who diagnosed with optic neurisis. I haven't heard about the illness before but according to her relatives, she suddenly can't see and her eyes slowly become blur. She currently had to walk with help because she can't see anything. Every month, she had to inject steroid into her eyes. At that time, she looked very tired after the injections. But now, I'll explain a little bit about optic neurisis.

What is Optic Neurisis?
The optic nerves carries visual information from your eye to your brain. Optic Neurisis is when your optic nerve becomes inflammed. It can flare up suddenly from an infection or nerve disease. The inflammation causes temporary vision lost that typically happens in only one eye. Those with Optic Neurisis sometimes experience pain. As you recover and the inflammation goes away, your vision will likely return. Besides, this illness doesn't always require treatment and can heal on its own. Medications, such as corticosteroids, can help speed recovery. Most who experience Optic Neurisis have complete vision recovery within 2 to 3 months, but it may take up to 12 months to achieve the vision recovery.

Who is at risk for Optic Neurisis?
- Female between the ages of 18-45
- Have been diagnosed with multiple sclerosis (MS)
- Live at high latitude

Finally today is the day. Hehe. 💃🏻💃🏻💃🏻 Today is our last internship life at Qalam Laduni Medical Centre. There were many experience that I have gain here starting from 6th February 2017 until today from management to customer behavior. The experience that I get can't found anywhere because this is a medical centre. Infact, thanks to the staff nurse because all this time, they helped us in many way including the medical terms and how to manage the patient in proper way. Not forget the practitioner who always there when I need help. I learned many things about the traditional medicine in practically because I don't have a chance before in UMK.
