Public health campaigns play a key role in combating syphilis. Posters, community outreach, and free testing events help raise awareness in high-risk areas. Campaigns target young adults, pregnant individuals, and underserved populations. By increasing visibility and education, these efforts help reduce the spread of the disease. Government and nonprofit groups like the CDC and WHO lead many of these initiatives.

Syphilis is caused by a spiral-shaped bacterium called Treponema pallidum. It is classified as a spirochete due to its corkscrew motion and elongated body. The bacterium enters the body through microscopic skin breaks or mucous membranes, often during sexual contact. Once inside, it can spread through the bloodstream and lymphatic system. Because it is very fragile, it cannot survive long outside the human body.

Syphilis develops in four stages: primary, secondary, latent, and tertiary. The primary stage begins with a painless sore (chancre) at the site of infection. Secondary syphilis presents with rash, fever, and swollen lymph nodes. Latent syphilis has no symptoms but can still cause internal damage. Tertiary syphilis, years later, can damage the heart, brain, and other organs if untreated.

Early symptoms of syphilis are often mild or unnoticed, which allows the infection to spread. The most characteristic sign is the chancre — a painless ulcer that appears where the bacteria entered the body. Secondary symptoms include a distinctive rash, often on the palms and soles, and flu-like symptoms. Without treatment, symptoms temporarily resolve but return in more serious forms. The bacteria silently cause damage during the latent phase.

Congenital Syphilis: A Preventable Tragedy

Congenital syphilis occurs when a pregnant person transmits the infection to the fetus. It can cause stillbirth, premature birth, or deformities in the newborn. Common signs in infants include rashes, bone deformities, anemia, and liver problems. Early testing and treatment during pregnancy can prevent transmission. Despite being preventable, congenital syphilis rates have increased dramatically in recent years.

If syphilis is not treated, it can lead to severe long-term complications. Tertiary syphilis can cause gummas, which are tissue-destroying lesions found in skin, liver, or bones. Neurosyphilis affects the brain and spinal cord, leading to confusion, poor coordination, or paralysis. Cardiovascular syphilis can weaken blood vessels and the heart. These effects may appear decades after initial infection and can be fatal.

Penicillin: The Gold Standard In Treatment

Syphilis is effectively treated with antibiotics, particularly penicillin. A single intramuscular dose of benzathine penicillin G cures early-stage syphilis. For late-stage or neurological cases, longer treatment courses are necessary. Patients allergic to penicillin may need desensitization or alternative antibiotics. Timely diagnosis and treatment prevent long-term damage and transmission to others.

Anyone who is sexually active can get syphilis, but certain groups are at higher risk. People with multiple sexual partners or those who do not consistently use condoms face an increased chance of infection. Men who have sex with men (MSM) account for a large proportion of new syphilis cases in many countries. Pregnant individuals are especially at risk due to the potential for passing the infection to their unborn child (congenital syphilis). Individuals living in areas with limited access to healthcare or STI testing are also more vulnerable due to undiagnosed and untreated infections.

The best way to prevent syphilis is through safe sex practices, including consistent condom use. Routine STI testing helps detect infections early, even when there are no symptoms. Pregnant individuals should be screened during early prenatal care. Public health education increases awareness and reduces stigma around STI testing. Reducing the number of sexual partners and knowing partners’ STI status also lowers risk.

From 2016 to 2022, the rate of maternal syphilis rose 222% in the United States, reaching 280.4 per 100,000 births in 2022. Increases in syphilis rates were widespread, rising for all maternal race and Hispanic-origin and age groups, prenatal care categories, and nearly all reporting areas. In 2022, variations in rates were observed across the characteristics studied. Syphilis rates were highest in mothers who were American Indian and Alaska Native, younger than age 25, and had no prenatal care. For 2021–2022, the rate ranged from 45.8 in Maine to 762.6 per 100,000 births in South Dakota.
