• Weekly or biweekly meetings in each school to discuss students with attendance issues and high-risk students.

Tier 2 Strategies:

• All students with asthma (current and active), life threatening allergies, diabetes, seizures or other medical health needs will have an IHCP created by the school nurse and kept in their paper records/EHR.

• The district will continue to staff a nurse care coordinator to support building-based school nurses with IHCP development equitably across the district.

Tier 2 Strategies:

• Staffing Nurse Care Coordinator and Attendance Case Manager/Social Worker.

Tier 3 Strategies:

• Scripts for nursing staff to use for phone communication

• Nursing part of parent/guardian meeting for students with health concern barrier noted.

• Obtaining Release of information for physician communication

Tier 3 Strategies:

• Tracking students that are absent and why. Contact home with caregiver to see if they can obtain MD notes for absences. Getting the school nurses coverage for the busy clinics so that nurses can attend attendance meetings. Nurses will track effective versus ineffective nursing interventions that have been tried as to whether the interventions work or do not work and share with nursing team.

Tier 3 Strategies:

• We have a NCC and a district social worker who are dedicated to addressing absenteeism in collaboration.

Tier 3 Strategies:

• Meeting with the head of school and other administration to review data and create a strong attendance policy.
