These documents follow through their SEND journey in school, a new one is started each school year or as and when the child is placed on Student Support.
The front page is an overview of information known by the class teacher or shared at transitions and includes any external assessments and recommendations about how best to support the child. It also includes the child's voice.
Assessment data is updated each term to track progress.
Targets are written in conjunction with the child and their parents/ carers, they are SMART and written in the voice of the child. Targets are reviewed termly with children and parents and new ones are written which are then shared with all who work with the child.
A learning support card may be written if it is helpful to keep beside the child.

Early Identification of Need

This document is to be used at the initial meeting with the parent/carer's to share concerns about the child. This meeting can be the result of the teacher or the parent/ carer outlining concerns.
The first page has a set of prompt questions to consider asking at the meeting which may support the identification of need.
The second page is a chance to discuss the strengths and areas of difficulty of the child and has sections for school, parent/carers and child to fill out. There then is a place for targets to be agreed upon for both home and school to focus on.
This document is between the class teacher, child and parent/carer and is not formally monitored. The review date can be determined on an individual basis. If after 2 or 3 cycles of review, concerns are still there or growing, then a discussion should be had with the SENCO to move to a Student Support Plan.

If you are considering whether a child would benefit from Connect Mental Health Team Support, we would use this consultation form - please speak to a member of SLT or SENCO for this.

From September 2026, schools will be able to make direct referrals to Connect using our new referral form.

This change is designed to make accessing support for children and young people simpler, quicker and more efficient, removing the need for a consultation before referral.

What you need to know

Schools can refer directly to Connect
A consultation is no longer required before referral
Parent/carer consent must be obtained before submitting a referral
Referrals will be triaged within 2 working days
Schools will receive the outcome following triage

Is Connect the right service?

Connect is an early intervention service and is best placed to support children and young people experiencing mild to moderate mental health and wellbeing difficulties, including:

Anxiety and worries
Low mood
Low self-esteem and confidence
Friendship difficulties
Emotional regulation difficulties
Other emerging mental health concerns impacting daily life

Connect is not a crisis service and cannot provide an immediate response to urgent, high-risk or emergency situations.

Where there are immediate concerns about a child or young person's safety, wellbeing or mental health, schools should continue to follow their safeguarding procedures and access the appropriate crisis or emergency services.

The new referral process

1. Identify a need

Consider whether the child or young person would benefit from early intervention support through Connect.

2. Obtain consent

Parent/carer consent is required.
Young person consent should also be obtained where appropriate.

3. Complete the referral form

Please ensure all sections are completed fully.

4. Submit the referral

Email the completed form to:

connectmhst@northtyneside.gov.uk

5. Connect triage

Within 2 working days, referrals will be reviewed and triaged.

Possible outcomes:

Accepted for intervention
Consultation recommended
Further information requested
Signposted to a more appropriate service

What if the needs are beyond the Connect offer?

The new referral form has been designed to closely mirror the MHEW Collaborative referral form, helping to reduce duplication of information.

Where we believe another service may be more appropriate, we will provide recommendations and guidance on next steps.

What about consultations?

Consultations remain an important part of our offer and can still be used for:

Advice and guidance
Complex or unclear presentations
Whole-school concerns
Cases where consent has not yet been obtained
Anonymous case discussions

A new Consultation Booking Form will be shared with schools during the first week of September.

Key reminders

Connect is not a crisis service.
Referrals are triaged within 2 working days and should not be used for urgent concerns.
Schools must continue to follow safeguarding procedures where safeguarding concerns exist.

We hope this new pathway will help schools access support more easily and ensure children and young people receive the right support at the right time.

Streamlining access to young people’s mental health and emotional wellbeing support in North Tyneside

We are pleased to announce that partners in North Tyneside have come together to form a Mental Health and Emotional Wellbeing Collaborative. This includes North Tyneside Council and Northumbria Healthcare NHS Foundation Trust.

What’s changing?

Most of the changes are behind the scenes. The collaborative is a multi-agency forum where professionals will come together to identify the best and most appropriate support for children, young people and their families.

For schools, you should continue to consult with the Connect Mental Health Support Team in your school (or centrally) in the first instance. Referrals into the HIVE Team will continue via the advice line. For alternative support, you will be able to refer directly to the new collaborative. The new request for support form can be found Young people's mental health | North Tyneside Council and should be emailed to mhew.collaborative@northtyneside.gov.uk.

For a limited period, any referrals via the existing CAMHS referral form will be sent onto the Collaborative. However, from Friday 30^thJanuary 2026, all requests for support must go through the Collaborative via the new request for support form.

Other professionals working with children and young people needing mental health and emotional wellbeing support can also refer into the new collaborative.

Managing referrals

The collaborative will meet daily Monday to Friday to triage all requests and make a joint decision on which service is best to support the needs of the child or young person and their family. The decision will then be confirmed in a letter to the referrer and the family.

Safeguarding and Crisis Support

The collaborative is not a route for safeguarding concerns. If you have a safeguarding concern about a child, please report it through the usual channels.

Should the child or young person be in an acute crisis and there is an imminent risk, please contact the crisis team via NHS 111 and select Option 2.
