They are used in public health programs (McGonigle&amp; Mastrian 2018).  Data can be collected about communities either from existing data, focus groups, statistics, health records, and making observations just from driving around the community. </div>]]></description>

         <description><![CDATA[<div>As nurses, we are always assessing our patients. We are constantly documenting risk assessments such fall and suicide risks. Nurses play a vital role in community health risk assessments also. Nurses can help identify certain needs of the community or risks that the community. At the very least, nursing documentation will help when data is being collected to identify risks and needs. When a plan is in place to provide education or share resources, nurses can help in implementing that plan. Nurses also can collaborate with other community agencies to provide input and feedback. </div>]]></description>
