• Attend an Introductory Meeting with the school team.

• Complete a Parent Input Form to highlight student strengths and areas needing support.

• Attend a School Visit Day with their child during the summer term.

For Current Teachers & SNAs:

• Complete a School Input Form to highlight strengths and areas needing support.

• Facilitate a Deputy Principal School Visit, where the DP can observe the student in their current class and see the supports in place.

• Support the student in completing a Communication Passport – a template is provided above or feel free to use a preferred template.

For Students:

• Participate in a School Visit Day to explore the new environment.

• Engage with Social Stories and Visual Supports to become familiar with the school routines.
