Ed. teacher is an afterthought in the IEP process.  They are often invited, but input isn't requested until the actual meeting where they are asked to describe how the student is doing in the class and then dismissed from the meeting.   I have developed a Google Form to ask for teacher input ahead of the meeting and for their input on potential accommodations/modifications.  I also added in the Google Form, potential topics the Gen. Ed. teacher can discuss this in the meeting, so they feel prepared.
