This service supports people for up to a three-year period and provides intervention to reduce the impact of the symptoms and support social recovery.          <description><![CDATA[<div>The main commissioners of services are the local authority and NHS. <br><br>Work in an integrated way through regular group meetings involving all partners, carers and those with lived experience.<br>National models and best practice models are used, however normally commissioners work locally to create provision relevant to the geographical and social area.<br><br><strong>Are there any gaps in service</strong><br>Yes particularly with specific mental health support for say those with a learning disability<br><br><strong>Role of social work<br></strong>To provide information on services locally and co-ordinate plans and services of support<br>Safeguarding<br>SW's feedback gaps in service to managers and commisioners. Since then we have developed a wide range of health and social care services for young people and adults with mental health needs, acquired brain injuries, eating disorders, autism and learning disabilities within the UK.<br><br>St Andrews - St Andrew’s Healthcare is a charity providing specialist mental healthcare for patients with some of the most complex, challenging mental health needs in the UK. We provide care across a number of <a href="https://www.stah.org/our-services/">services</a>, including <a href="https://www.stah.org/our-services/mens-mental-health/">Men’s Mental Health</a>, <a href="https://www.stah.org/our-services/womens-mental-health/">Women’s Mental Health</a>, <a href="https://www.stah.org/our-services/camhs/">Child and Adolescent Mental Health Services (CAMHS)</a>, <a href="https://www.stah.org/our-services/neuropsychiatry/">Neuropsychiatry, </a><a href="https://www.stah.org/our-services/autistic-spectrum-disorder/">Autistic Spectrum Disorder</a> and <a href="https://www.stah.org/our-services/learning-disability/">Learning Disability</a>, with the majority of our 600 patients referred to us via the National Health Service.<br><br><br></div>]]></description>

         <title>Tiered provision of services</title>

         <description><![CDATA[<div><br><br><br>...from April 2021, the NHS has started rolling out a programme called the Integrated Care System which will change the landscape of the current healthcare system. See the below section, ‘What is Integrated Care Systems?’ for more information.</div><div><strong>Primary care</strong>. This is the entry level of care for the NHS. You can get treatment and support from primary care services if you have mild or moderate mental health problems. Primary services include:</div><ul><li>your GP,</li><li>your local link worker through your GP. See the later section below for more information, ‘What is social prescribing?’ and</li><li>talking therapy from your local Improving Access to Psychological Therapies (IAPT) service.</li></ul><div><strong>Secondary care</strong>. This is the next level of care in the NHS. It covers general community and hospital care.</div><div>A mental health team (MHT) is part of community care. Community care means you get treatment outside of a hospital stay. You may be referred to an MHT if you need more support with your mental health. MHT’s are staffed with professionals such as a psychiatrist and psychologists.</div><div>See the next section, ‘Are there different mental health teams?’ for more information.</div><div>Hospital care includes inpatient treatment. You are an inpatient in hospital if you stay voluntarily, or if you are detained under the Mental Health Act.</div><div><strong>Tertiary care</strong>. This is highly specialist care within the NHS. It covers specialist community and hospital care.</div><div>Examples of community tertiary care are:</div><ul><li>assertive outreach teams, and</li><li>specialist national services within the Maudsley hospital in London.</li></ul><div>The Maudsley is a national service which means that you can access it even if you live outside of London. But you will need a referral.</div><div>Specialist hospital care includes secure units. Patients in secure units are usually detained under the Mental Health Act and present a level of risk greater than general mental health services could safely deal with.</div><div>What is the Integrated Care System?</div><div>Integrated care is about removing traditional divisions between different tiers of care such as primary and secondary care. This is so that people can get the support that they need. It aims to remove divisions such as those between:</div><ul><li>hospitals and GPs</li><li>physical and mental health and</li><li>NHS and the local authority.</li></ul><div>These divisions have meant that lots of people have had disjointed care.</div><div>Integrated Care Systems are new partnerships between the NHS and other health and care organisations. Such as the local authority, voluntary sector and social enterprise sector. By working together and putting you and your needs at the centre, your care will have a more collaborative approach.          <description><![CDATA[<div>Clinical reference groups provide clinical advice and leadership on the specialised services in mental health. These groups of clinicians, commissioners, public health experts, patients and carers use their specific knowledge and expertise to advise NHS England on the best ways that specialised services should be provided.<br><br></div><div>CRGs lead on the development of clinical commissioning policies, service specifications and quality standards. They also provide advice on innovation, horizon scanning, service reviews and guide work to reduce variation and deliver increased value. CRGs, through their Patient and Public Voice (PPV) members, also help ensure that any changes to the commissioning of specialised services involve patients and the public.<br><a href="https://www.england.nhs.uk/commissioning/spec-services/npc-crg/group-c/">NHS commissioning » Mental health (england.nhs.uk)</a><br><br>As a profession, social work has always played a key role in managing risk and complexity, working with people with the most profound and enduring health and social needs and who are often the most socially excluded and at risk of harm. Social workers will continue to support people in crisis. However, as we move towards greater integration of health and social care with a focus on prevention and wellbeing to reduce demand for more intensive services, we have a unique opportunity to reposition social work at the heart of person-centred adult social care.
