Who else to talk to?

GP discuss health of Kirsty and Betty, any contacts recently? Any concerns?

SW – Support needed? Who is covering the caseload while on leave? What contacts have they had? Can they make contact? Assessments needed? Has Kirsty been advised of Claire’s law? 

Police? Any reports of DA situations? Has Kirsty enquired about Claire’s law?

MH teams – any contacts? Referrals? Can a referral be made?

Support workers from other agencies – who else is involved or could be involved?

 

Barriers to collaboration:

Different agencies have differing agendas, priorities, workforce limitations, knowledge and skills– working across boundaries can be difficult but is achievable for the care and support of joint patients. Multi-disciplinary person- centred approach is safer and reduces risks.

 

Improving collaboration:

Person- centred approach is achievable with MDT meetings, and good clear communication. An understanding of each other’s roles and responsibilities. Regular MDT and care planning meetings are essential especially with complex cases.
