Saga Care Helapuisto is a private sheltered home including a special ward called Helakoti for 12 people with severe memory disorders. During my first practical training period, I got to work at both sides of the building. Working at this kind of a modern (built 2012) home facility for the elderly was a real experience. I would easily see myself living in this kind of a sheltered accommodation in the distant future. Even for bedpatients I think there would be benefits and opportunities for social life – the downside is, the rent is around one to two thousand euros per month according to your need for services.

This video is part of Saimaa university of applied sciences paramedic students' thesis. I have simply translated it in English as best I could.

Place the manset on the patient's bare skin.

Make sure the tubes of the manset are pointing towards the patiens's fingers on the inner arm.

Palpate the pulse on arteria radialis (wrist arthery) and arteria brachialis (brachial artery).

When you have found these, you'll know both the place to palpate and the place to auscultate.

Place your fingers on the arteria radialis and start pumping until you cannot feel the pulse any more.

After that, pump ca. 20-30 mmHg (mm of mercury) more.

Place the bell part of your stethoscope on the spot you found on the patient's brachial artery.

Turn the screw on your meter so that the pressure will slowly start to descend.

A suitable pressure drop is only a few mmHg per second.

When the pressure drops to the level of the patient's systolic blood pressure, you shall hear a low pulse on your stethoscope.

The moment you hear the first pulse is the systolic pressure as shown by your meter.

When the pressure drops the patient's blood will start to circulate also during their diastole and the auscultative pulse will deteriorate. When you notice the auscultative pulse completely disappering, that is the measurement for diastolic blood pressure.

Remember to let all the air out of the manset after you get both measurements.

If you need to know the systolic blood pressure from the patient really fast, you may measure it by palpating:

Place your fingers on the arteria radialis and start pumping until you cannot feel the pulse any more.

After that, pump ca. 20-30 mmHg (mm of mercury) more.

Turn the screw on your meter so that the pressure will slowly start to descend. When you start to feel the pulse again, that's the measure of the patient's systolic blood pressure.

https://vimeo.com/218830007

III - Instructing the patient

Home care instructions for Hypertension

Hypertension is most commonly known as High Blood Pressure. Most of the time, there are no symptoms, but when high blood pressure goes untreated, it damages arteries and vital organs thoughout the body. That's why it is often called the "silent killer". Globally, 9,4 million die every year of high blood pressure. It is the biggest single risk factor for death world wide causing heart disease, stroke and kidney disease and diabetes.

Blood pressure is a measurement of the force applied against the walls of the arteries as the heart pumps though the body. The force and amount of blood pumped, and the size and flexibility of your arteries determine your blood pressure number. A reading consist of two numbers, for example 120/80, which is read as "120 over 80". A clinic measurement less than 140/90 is generally considered healthy for an adult. Blood pressure measured at home is usually lower than levels recorded by a doctor, so self recorded blood pressures below 135/85 are generally desirable.

You can prevent stroke, heart and kidney dieases by treating your high blood pressure at home. Please be sure to take into account all the instructions below.

1. Take your prescribed medication regularly as instructed. Report any side effects to your physician immediately.

2. Measure your blood pressure regularly. You can do this at home with an automatic cuff style upper arm monitor.
- Be sure your cuff fits. Measure your upper arm before you purchase a machine.
- Be calm. Do not dronk caffeinated beverages or exercise within 30 minutes before measuring your blood pressure
- Sit quietly for 5 minutes without talking to anyone before and during the measurement. Sit with your back straight and supported. Do not cross arms or legs and keep your arm supported with the upper arm at heart level.
- Take multiple readings each time you measure, 2-3 tims one minute apart and record all the results.
- Measure your blood pressure at the same time every day. Recommended times are first thing in the morning and again in the evening.
- Keep record in a book you can keep with you and show your health care provider. Seek advice with your doctor if your blood pressure is regularly not within normal ranges.

3. Live healthy. Pay attention to three keys of healthy living:
- Take a habit of measuring your Waist Circumference (WC) and Body Mass Index (BMI). Overweight is linked to hypertension.
- Pay attention to what you eat. Do not skip meals. A healthy lifestyle is not a diet. Limit portion sizes, eat slowly and listen to your body. Choose a lower salt option, read the labels and try to avoid processed and fast foods. Increase your fruit and vegetable intake and limit your alcohol intake to no more than one standard drink for women and two for men.
- Live an active life. 30 minutes of recommended daily exercise can consist of 3 times 10 minutes! Walk up the stairs at work, park your car or take the bus not all the way so you have to walk a block or two. Dance, go for a walk, play with your kids or grandkids or neighbours outdoors. If you want to do weights check wih your physician before starting.

http://www.interimhealthcare.com/InterimHealthcare/media/Interim-Public-Site/Home%20Care%20Section/WHD-2012-brochure.pdf

http://www.kaypahoito.fi/web/kh/suositukset/suositus?id=hoi04010
