Challenge: Usually the last to leave a classroom and the last to get to the next class.

Being in the 4th grade, students do not usually receive a break in between classes and transition from one class to the next without giving Sofia time to adjust to the transition nor to get herself settled in. Leaving the classroom last, probably means she is not given enough time to pack her belongings and complete any assignments she needs to before she leaves the class. A solution to this may be adding on another accommodation to her 504 Plan that states she would get an early transition from one class to the next so that she has time to organize herself and mentally prepare herself for the next class.

Challenge: Teachers often see her in her own world where “her brain will freeze” as one teacher described it

When Sofia is in her ""own world", teachers must remember it is the "Inattentive" part of her disability. Perhaps something that would decrease that is giving her a class job so that Sofia is not given the opportunity to space out and lose out on learning.

Some class jobs could be, handing out papers, time keeper, write on the board etc.

Challenge: Difficulty attending to tasks.

With Sofia not being able to attend to tasks, she is unable to complete her full assignment, which is probably related to her "zoning out" or not fully understanding what the task is asking of her. Perhaps chunking the assignments would help her feeling more accomplished when she does complete a section.
I realized that her 504 plan was written in in the 2nd grade and modifications should be added on to each area of concern. Modifying Sofia's work by giving her less work to complete will probably allow her to be more successful in the classroom.

Challenge: Trouble with spelling and fluency.

With Sofia being an ELL student with ADHD, it is more difficult for her read like her classmates or with a certain level of fluency. Based on Sofia's writing sample, she can write complete sentences. Perhaps giving Sofia a mini reference book with words she does not know how to spell would help her.
Perhaps also increasing her ELL services until she becomes more fluent in english.
Sofia should also be encouraged to attend tutoring after school or during the school day for further supports.

Challenge: Math, spelling, and language arts grades are well below grade-level.

Manipulatives could be used for further success in Math class. Anchor charts and reference sheets should be a part of her IEP/ 504.

As an English language learner, Sofia would benefit from extra time on assessments with sentence starters to help her thinking. Providing her with a criteria for success is also something she could benefit from during classwork as well assessments.
