         <description><![CDATA[<ul><li>Consent from parent has been obtained&nbsp;</li><li>45-60 days to get evaluation done</li><li>Specific evaluations are completed by the appropriate person.&nbsp; (i.e. General intelligence evaluation is done by&nbsp; school psychologist, Academic achievement evaluation is done by special education teacher)</li><li>2-4 observations will be taken (observations can be completed by school administration, school phycologist, or special education teacher)</li><li>Parent, student, and general education teacher interviews will be done</li><li>Student records will be reviewed</li><li>General education teacher will continue to collect data and make notes of new&nbsp; accommodation and strategies used.</li></ul>]]></description>

         <title>Phase 5: Eligibility </title>

         <description><![CDATA[<ul><li>Meeting will be set to determine if student is eligible for special education.&nbsp;</li><li>A notice of 10 or more days needs to be given to parents prior to date of meeting</li><li>All parties in the team presents data, provide copies to parents, and answers any question and concerns parents might have.&nbsp;</li><li>All parties in school team discuss if they agree student meets the two prongs that will determine if student is eligible for special education services. (two prongs are, 1.) Does the data support the student has a disability according to IDEA? 2.) Does the data support that the student needs specially designed instruction?).</li><li>Every party must agree that the student meets the qualifications of both prongs.<ul><li>If all parties do not agree, a 504 plan is looked at for student</li><li>If all parties do agree, a meeting date is set to discuss an IEP. </li></ul></li></ul><div><br></div><div><br></div><div><br></div><div>&nbsp;</div>]]></description>

         <title>Phase 6: Initial IEP Meeting</title>

         <description><![CDATA[<ul><li>Meeting must occur 30 days after eligibility meeting (but usually done within 5-10 days)&nbsp;</li><li>Must have a parent or guardian there, one GenEd<br>teacher, SpEd teacher, administration or LEA,<br>school psychologist, any related service provider,<br>any third party the parent chooses to attend, and<br>the student if they are 14.</li><li>Team meeting notes are taken, and parents voice their concerns. The school has to address how they will address the parents concerns.&nbsp;</li><li>If parents agree with IEP, they will sign it.</li><li>The day the IEP is signed, is the day its implemented.<br>&nbsp;</li></ul><div><br></div>]]></description>
