This support can reduce feelingsof isolation, offer valuable resources, and encourage socialconnection, all of which contribute positively to both the child’sdevelopment and the mother's mental health and confidence.</p><p><br></p><p>Practice nurse - </p></li><li><p>Practice nurses play a vital role in the long-term management of chronic conditions by providing ongoing support and monitoring. They help with routine checks, such as taking blood samples, monitoring vital signs, and assessing the condition of feet in patients with diabetes. They also educate patients on managing their condition, including offering advice on diet, lifestyle changes, and medication adherence. This consistent care helps patients maintain control over their health, reduces complications, and improves overall quality of life.</p></li></ul>]]></description>

         <description><![CDATA[<p>Group 2 </p><p>Liam</p><p>The paediatric nurse is very important in supporting both the child and their family. They teach the child and parents how to manage the condition, such as using an inhaler correctly, and provide reassurance to reduce anxiety. By creating a care plan for school, the nurse helps keep the child safe and supported outside the home. Overall, they give both medical and emotional support, helping the whole family feel confident in managing the child’s health.</p><p>Also the multidisciplinary team would include </p><ul><li><p>GP / Paediatrician : to diagnose, prescribe medication, and monitor Liam’s asthma.</p></li><li><p>Paediatric Nurse / Asthma Nurse Specialist – to teach inhaler use, create care plans, and provide ongoing support.</p></li><li><p>School Nurse 0r teachers  – to make sure staff at school understand his condition and know how to respond in an emergency.</p></li><li><p>Pharmacist – to provide inhalers, spacers, and advice on medication use.</p></li><li><p>Dietitian – to support healthy lifestyle choices, as diet can influence asthma triggers.</p></li><li><p>Physiotherapist / Respiratory Therapist – if Liam has breathing difficulties or needs help with physical activity.</p></li><li><p>Family / Parents – central to his care, ensuring his condition is managed consistently at home.</p></li></ul><p><br/></p>]]></description>

         <description><![CDATA[<p>The midwife balances medical care with emotional support for new mothers by, providing holistic care which they then view pregnancy and childbirth as a significant life event. Midwifes also have active communication skills which allows them to communicate with the mother. Midwifes are trained to actively listen which builds a safe place for mothers to express any concerns they may have. As well as building a strong relationship with the mother, midwifes are also able to provide information about labor to the mother so they have a better understanding of things. Midwifes not only help deliver the baby, but also are there for the mothers emotionally helping them get through it calmly and comfortable. </p>]]></description>

         <description><![CDATA[<p>Children’s (Paediatric Nurse). Liam is age 7 and has asthma. The role of the Paediatric Nurse’s is important because he needs the full instructions of how to use an inhaler as it’s a medical prescription. If he doesn’t know how to use one it would be dangerous for Liam, possibly leading him to asthma attacks. </p><p><br/></p><p><strong>Learning Disabilities Nurse. </strong></p><p><br/></p><p>Sam is 22 and has down syndrome. He struggles with attending medical appointments. Learning Disabilities nurses are very effective at reducing many person-level and professional-level barriers to healthcare access, especially through, communication support, and staff training. For sam, the nurse could support them and try be understanding which will make sam feel comfortable and secure in his appointments. </p><p><br/></p>]]></description>

With physical help the nurse could help Singh by monitoring the wound and this is important as it can prevent life alerting complications like infections etc. Emotionally,  the adult nurse could encourage mobility and make Singh motivated to walk by celebrating small victories etc. </p><p><br/></p><p>The Mental health nurse coukd ensure that Jade is taking her medication on time and therefore she is okay. Furthermore, The nurse could suggest certain websites to Jade to keep her mind occupied and to ensure she has someone to open up to.
