To form the passive voice, one takes the verb in question and converts it to the past participle. Then you add the verb to be in the conjugation and time corresponding to the new subject.
For example:
  • The police rescued the boy.
  • The boy was rescued by the police.
The first sentence is in active voice. The second is in a passive voice.
The verb in the first sentence (rescued) becomes "was rescued". Let's remember the rule: You take the past participle of the verb and add the verb to be conjugated in the corresponding tense (in this case the past tense) and for the corresponding person (the boy). 
The subject of the first sentence (the police) becomes the agent complement in the passive voice, adding by in front. The agent complement is not always placed in the sentence. It is only added if it is relevant.
The passive voice is often used in formal texts. Switching to the active voice will make your writing clearer and easier to read.
  • A great deal of meaning is conveyed by a few well-chosen words.
  • Our planet is wrapped in a mass of gases.
If we want to say who or what performs the action while using the passive voice, we use the preposition by. When we know who performed the action and are interested in him, it is always better to switch to the active voice instead.
  • "A Hard Day's Night" was written by the Beatles.
  • The movie ET was directed by Spielberg.
