Welcome to the Leeds Centre for Newborn Care

We have two neonatal units;

- a big neonatal intensive care unit (NICU) which looks after extremely preterm infant, and infants with cardiac, surgical and neurosurgical problems. This is L43, Clarendon wing at the Leeds General Infirmary

- a special care baby unit (SCBU) which is J01, Gledhow wing at St James Hospital.

Leeds has approximately 9 000 births per year.

We also look after babies born all around the region who come to us for specialist care for heart conditions, surgical problems, liver and kidney problems and respiratory problems. We are a busy unit and care for many babies with very complex problems along with our colleagues from the paediatric specialist services.

It is a busy job but you will learn a lot and will have the opportunity to experience many things that you may not see in other neonatal units.

The more you put in, the more you will get out of it.

We hope you enjoy your time with us on the neonatal unit.

The neonatal consultants

We hope that you find induction helpful and that it will help get you started on how the neonatal unit works and what we expect.

There are many useful resources on this Padlet site to explain how things work and to increase your knowledge.

During your time here:
1. Please ask if you don't know something. As well as delivering a service and looking after babies we are here to teach you.
2. If you don't know the answer to something ( be it from a doctor, nurse or parent) say so and find out the answer
3. Always do things in a timely manner (we are delivering intensive care!)
4. Look after every patient and family to the best of your ability- imagine every baby is your own child or niece/nephew etc
How would you want to be treated?

Intro to the rota

Each week, the rota is sent out for the following week showing which doctors and advanced neonatal nurse practitioners (ANNPs) are working in each areas e.g. SJUH, ITU, HDU, Surgical newborns (SNB) etc.

Working in intensive care is challenging and the babies we care for are very vulnerable. They benefit from continuity of care and this also provides a good training base for the registrars on the unit.

It is also important to be able to take annual leave and attend any training that would be beneficial. Please request your annual leave as soon as possible.

The rota has been designed to maximise your annual leave, with unallocated short days placed within weeks with rostered off days.

We have a rolling rota on the neonatal unit covering both the LGI and St James’campuses. Whilst at St James there is only one registrar on for the day and night, but at LGI there can be up to 4 on during the day. The primary roles for the daytime HSTs at LGI are:

· ICU Service Registrar

· HDU Service Registrar

· Support Registrar

· PNW and clinic Registrar

The HST rota runs alongside the ANNP rota. There will be at least one ANNP working clinically each day, and they rotate through ICU, HDU and the PNW on a weekly basis. Wednesdays at St James are routinely covered by an ANNP.

More information about roles can be found in the guideline for admission to the neonatal service and is described below.

However, they may be moved to wherever the workload is highest.

They hold the registrar delivery bleep and will lead the team at high risk deliveries. They are responsible for the safe transfer of the infant to the neonatal and the initial stabilisation such as line insertion before the baby is handed over to the ICU or HDU teams.

When an ANNP is based in HDU they will be holding the delivery bleep Monday to Friday, but may still require the presence of the Support Registrar depending upon the nature of the delivery.

Another role of the support registrar is to facilitate the transfer of infants to radiology, surgery or cardiac catheter lab if a medical escort is necessary.

In addition to providing cover on ICU/HDU, there will be occasions when the PNW and outpatient clinic cover (Tuesday pm and Thursday am) is more needing of support. This will primarily be on days of sickness or when the PNW reg slot is vacant.

ATAIN (Avoiding Term Admissions into Neonatal units) is a NHS national initiative that involves reducing unnecessary admissions of term babies to the neonatal unit. Although the overall term birth rate has reduced over recent years, the national admission rates have increased. Few babies born on time should require neonatal services and separation of mother/baby can have a lasting impact. The initiative focuses on several key areas of care to try and reduce admissions. The areas are as follows:

Respiratory conditions

Jaundice

Hypoglycaemia

HIE

Here is a link to an online learning package (https://www.e-lfh.org.uk/programmes/avoiding-term-admissions-into-neonatal-units/
