         <description><![CDATA[<div>10.2.21 - 11.00am - Client interviewed for first time in the office. Client referred by Child Protection agency. Client seemed very angry and hostile at the commencement and seemed to settle in quite well once I informed her of her rights and that this was voluntary. Client advised she has had numerous involvements with agencies over the years, some of which were okay to deal with and others that had a negative impact. She stated "I feel like everyone else seems to be in control of their lives except me." Her parents and boyfriend have been "giving her grief" about using drugs. Client advised 🤬 is her current drug choice and she is restricted to solely taking it "fairly heavily" on weekends due to financial constraints. She said she is "struggling to get through the week." I outlined the AOD policy around use of drugs on our premises and the implications of such should she be found using on the premises including being removed from the program. Client has agreed/consented to come back next Friday 20 Feb at 2pm to discuss her issues in further detail and will then make a decision around whether she can commit to participating in the program.  </div>]]></description>

         <title>11.0-11.20am-Client referred to from local child protection agency to be interviewed for the first time in office. Client seemed uncomfortable and hostile at first but settled in ok once I informed her of her rights and advised this was done on a voluntary basis only. Client has had other involvements with agencies over the years with some having a negative impact on her. She is feeling as though she has no control over her life at the moment and her parents and boyfriend are &quot;giving her grief&quot; about her drug use. Client advised her current drug of choice was 🤬 and due to financial restraints uses it &quot;fairly heavily&quot; on weekends. She mentioned how she is struggling to get through the week, I advised her of the AOD policy and about the use of drugs on our premises and she advised me she would leave the program if she uses it on the premises and has agreed to return next Friday 20 feb at 2pm to further discuss in more detail and other issues and to make a decision  in regards to committing to this program.</title>

         <description><![CDATA[<div>Case notes<br>10/02/2021 – 11:15 AM – Main Office<br><br></div><div>On the 10.02.2021 the Client attended an interview in the main office.<br> Client was referred by Child Protection Agency.<br> As observed, client is very angry and hostile at the beginning of the interview but settled once it has been indicated her involvement is on a voluntary basis, and we could terminate the agreement at any time. As per her file, it shows actions have taken between the Client and the service. Some of them have been successful other had a negative impact on the client and her wellbeing.<br> The clients state she feels she is not listened to and is not in control of what is happening to her. Clients is having problem with her parents and her boyfriend about her drug consumption.<br> Client have also state that 🤬 is the drug she currently uses and is only limited to weekends because of financial restraints. (client).<br> Clients circumstances are “fairly heavy” during the weekends and she struggles to get through the week.(client)<br> I outlined client policy regarding AOD while in the service and that she will have to leave the program if she breaches it.<br> She has agreed to come back next Friday at 2:00 pm for more details and to decide  if she wants to commit to the program.<br> Pia Martinez.<br> Case manager. </div>]]></description>

Client stated that he/her family is unable to assist with transportation. He/her states “I had been in contact with local community transport group who are happy to assist”. However, client will need to contact the community transport to arrange and provides information regarding dates and times. Client agreed to contact the community transport within the next few days. Client states “I would see him next week as agreed”.<br><br></div>]]></description>

The client is going to determine the level of support needed and provide the transport group with the time and dates for his appointments and he has informed me that will contact the transport group in the next following days.          <title>10/02/2021  11.25AM  Spoke with client over the phone to check in on problems with transport to visit various specialists at the local hospital. Clients family are unable to assist. Advised client local community transport group are happy and able to assist and that client will need to make contact with the local community transport group to arrange dates and times that the client will be visiting the specialists at their local hospital. Clients response is they will be in contact with them in the next few days to make arrangements and was fine to do so. Arranged meeting with client next week to see him. </title>

         <description><![CDATA[<div>Date        : 10/02/2021<br><br></div><div>Time        : 11.11 am <br><br></div><div>Venue     : Office<br><br></div><div>Prepared :  by  Maureen .<br><br></div><div> <br><br></div><div> <br><br></div><div>I made a follow up through the phone  with a client who was having transport issues to get to his local hospital to see various specialist.<br><br></div><div>He communicated to me that he was still facing the same challenges and that his family were unable to assist in the matter.<br><br></div><div>I told him that I had communicated with the local community transport group and they were happy to assist him with the issue as long as he contacts them and make early arrangements by providing details of dates and time of his appointments at  the hospital.<br><br></div><div>The client agreed to it and assured me that he was going to get into contact with the local community transport group in the next few days.<br><br></div><div>I also reminded him of our next meeting which is scheduled for next week <br><br></div><div> <br><br></div>]]></description>

Client was referred from the local child protection agency. Client Seemed very angry and hostile at the beginning of the interview but seemed more settled once I had explained that her involvement with the agency was to be on a voluntary basis only and she has the rights to choose to leave whenever she wanted. </div><div>According to her feedback, she Stated that she had a number of involvements with other agencies over the years, some of which she seemed OK about, some of which seems to have acted as negative reinforcement.</div><div>Client Stated "she feels that everyone else seems to be in control of her life except her". She also stated that her parents and boyfriend have been 'giving her grief' about her using drugs. She also informed that 🤬 is her current drug of choice and that her use is limited to weekends because of financial restraints.</div><div>Client Stated that weekends are 'fairly heavy' and that she is struggling to get through the week.          <description><![CDATA[<div>at 10.45pm interview took place with client at the office, for the first time. my client was refered by DHHS child protection. during meeting clients behaivour seemed aggitated and she was very angry, i emplained her rights and that this was a volentary to which she then calmed down and settled.<br>client has informed she has had many involvements with agencies over the years causing negative impact and has expressed she feels as though "i feel like everybody is in control pf their lives except me"<br>she expressed her family members and partner have given her grief over her drug use.<br>client has advised her drug choice to be 🤬 and she has restricted herself to taking it regulary and only on weekends due to her financial strain.<br>client expressed her struggle during her weekdays.<br>i have outlined and explained the AOD policy around clients drug use within the premisis an if this is breached she will be removed from the program.<br>i have booked client in on friday the 20th febuary 2021 at 2pm for an appointment in which she has agreed to attend and discuss her issues in further detail and decide the process and commitement to the program. meeting ending 11.45am.<br><br></div>]]></description>

This weekend I was referred a client with a history of substance abuse issues and extreme hesitation of using the service as they have had some negative experiences with agencies over the years, that have been impactful on the client. 

After a thorough discussion, the client has realised that involvement with our agency is purely voluntary and there to assist her. We have arranged an appointment next Friday at 2pm to discuss and address  the issues of concern and if the client feels comfortable enough to commit to the program and our guidance to reach her goals.]]></description>

         <description><![CDATA[<div>Date and time: 10<sup> </sup>February 2021 @ 10am <br>Event: Interview with: Jane Roberts and case worker:<br> Shelley Cross. </div><div>Location: Head Office: 1 Fitzroy Street, Fitzroy  <br><br></div><div>Client attended the head office today for her first interview with writer, as referred by the local Child Protection Agency.  Upon arrival the client appeared to be angry, writer informed the client that attending the interview was voluntary and advised the client she had the option to leave at any time if she chose to.  Client appeared to settle down after writer had explained this to her.   Client advised writer she has been to several agencies over the years and some have been positive, and others have been a negative experience.   Client stated during the interview she felt everyone else seems to be controlling her life, and not herself.  Clients told writer that her parents and boyfriend have been giving her ‘grief’ about her using and stated to writer that she is currently using 🤬 and limits her use to weekends at this present time due to financial restraints.   Client advised that on the weekends her usage is ‘fairly heavy’ and that she is struggling to get through the week.   Writer explained to the client during this interview the policies regarding a<br>alcohol and drug use on the premises, and client advised that she will leave the programme if she uses alcohol or drugs on the premises.   Client agreed to come back to the centre on 19/02/21 at 2pm to discuss with writer the current issues in greater detail, and to advise writer if she wishes to commit to this program.  Interview concluded at 11am.     </div>]]></description>
