The versatility of this assessment adds value to any lesson.
Students will write down:
Three things they learned during the lesson
Two things they found interesting about the lesson.
One question they still have about the material learned.
The purpose of the assessment is twofold; One to recognize what the children learned and still have questions about, and two for teachers to evaluate the effectiveness of their lesson.

This assessment can be completed before, during, and after a lesson to evaluate a student's understanding as well as any previous background knowledge or skills they may bring to a topic. 
The added bonus of the KWL +H chart is that students can help describe how they learned the topic and explain the topic back to the teacher. Teachers can use this information to perhaps tweak future lessons for each individual student or create remedial materials.

This assessment should be prefaced with a plea for honesty. When students are honest with their understanding and self-scoring of taught materials, they benefit themselves and their teacher. The self-evaluation tool can also be used to capture efforts that are not otherwise seen, such as helping a peer or tried a different strategy and still came to the same conclusion.  The Self Evaluation tool can also be used for all subject matter.

Teachers can use this to pose a question, check for understanding, or have students write about what they still need reviewing. From research, I learned that this could be a versatile assessment or temperature check for busy classrooms that do not allow individual student feedback during lessons. Quick writes can be used in all subject areas.

I find that some students struggle to identify specific text phrases to support the author's opinion. The opinion-proof assessment will help teachers identify the students that are unclear on the author's point of view, central point, and opinion. They can also use this tool for paraphrasing and quotation accuracy and remediation identification, or perhaps identify a minilesson topic for the entire class.

The versatility and editability of this tool are endless. Teachers can use the data collected to identify trends throughout their student population for remediation, minilesson identification, or teacher performance.

The feature I like about this form is that it forces the students to rewrite the I can statement. This focuses students on the task that is being evaluated and holds students personally accountable for their understanding. Honesty should be expected, and teachers should explain to students how they are using the tool. Teachers should also READ the student's reflections to craft future lessons and create remedial sessions. The Summary activity can be time-consuming and require a thorough explanation of expectations. Useful student feedback will require the early implementation of this activity. Once students have been introduced to the 1-2-3-4 method of understanding, teachers can quickly evaluate students understanding of current topics and concepts. Teachers use this technique by asking students to put up their fingers and tell them silently their level of understanding. The early introduction of this exercise is critical in the effectiveness of this assessment. This assessment takes the next step by asking students to guess the answer based on what they have learned. The I think portion of the assessment can help teachers evaluate their understanding and get a glimpse into how they may have arrived at their question. Teachers can use this assessment for all subjects and help start student conversations that are culturally responsive and interesting to the individual student. (This is one of my favorite assessments)
