Lapses in food hygiene discovered: 
  1. Cockroach infestation
  2. Poor food hygiene practices
    • Raw seafood chopping board was found directlu stacked against chopping board meant for RTE food; RTE food was kept in proximity from raw chicken in the walk-in chiller

Action taken: Suspension for 9 days and composition sum of $400
