Any cuts, bruises, falls, etc. need to be reported immediately by the staff member on duty on a District Student Accident Form. Remember, when in doubt, fill out an accident form. You may ask the school secretary or Principal for assistance if needed. Whenever possible, use the exact words of the student, or your own precise observations. Examples: John said, "I ran into the basketball pole when I was looking at the volleyball game." "Sue hit me in the mouth and broke my tooth." The completed form should be turned in to the office as soon as possible, and always no later than on the day of the accident.
