First one, for sure, it's the Sybil's childbirth and death. The second is the arguments over christening. 
The third scene, the one that interested me most, is the controversy of Dr. Clarkson and Sir Philip Tapsell and background conflict between Branson and Lord Gratham.

While it is known that Dr. Clarkson made several medical mistakes in the past, as he was the doctor who misdiagnosed Matthew and Lavinia, he is still a professional, and Lord Gratham just dismissed his professional opinion, which results in Sybil's death from eclampsia. Also, despite Sir Tapsell had been granted a knighthood for his success in his work field, he was at least unprofessional in rejecting his colleague's diagnosis, and he was outright rude doing so. 

As two doctors fight over diagnostic, the whole family splitted over between Branson and Lord Gratham camps, who have a disagreement over doctors and their diagnosis. Naturally, when both Lord Gratham's wife and the oldest daughter, and after that even his mother went against him he took it personal and, using his position as head of house, overrode all protesters. In result, Sybil is dead, little Sybil is an orphan now, and Branson is a widower, family and servants are depressed, but what exactly caused all these events? Lack of good name, lack of professionalism and bit of hubris from all sides of the conflict.
