It's her main goal since a long time; becoming a Pretty and finally be with Peris. She's even ready to betray Shay to have what she wants. Her appearance comes first. Tally is selfish because she only thinks about herself. 

Shay : She's really a loyal friend. Before leaving, she took the time to go see Tally and offered her to come with her. She made sure she would be okay. Shay is good influence to Tally because she doesn't want the surgery, she's okay with the fact of staying "ugly". All her life isn't based on this surgery. 

Peris : He already had the surgery, so he's one of the major reason why Tally wants the surgery. The surgery changed him not only physically but mentally too; when Tally went to see him in New Pretty Town, it was like he was ashamed of Tally, because she was an Ugly.

In the dorms, people are focusing on behaving on the rules and only wait for the moment that they will become pretty. Plus, in New Pretty Town, everyone is based on the idea to be "perfect".
At the Smoke, being perfect is probably the less important issue.

-When she decides to leave to go after shay, and do what Dr. cable tells her to do.

-When Tally is at the final step of Shay's direction. It means that she's almost there and Dr. Cable and her group will know where. Tally realizes that she's betraying her friend, but tells herself that she made a promise to Peris before making that promise to Shay.

-When she doesn't tell them about the tracking device in the necklace. The Uglies who live in Smoke town found before a tracking device in Tally's hoverboard.

One of the first conflicts are with Dr. Cable who wants her to lead the "special circumstances" to "the smoke", and if she doesn't do that, she will never become pretty.
conflict = Tally has to decide which is more important : becoming pretty or staying loyal to Shay.

2. The second one is when Tally arrives at "the smoke", and doesn't tell them the truth about the tracking device in her necklace. 
conflict= It's based on loyalty once again, perhaps this time is to the "special cricumstances". 

3. third, Tally is having all the time 2 thoughs, becomeing pretty or staying an ugly for therest of her life. I think that it's like two angels, the good one and the bad one. -Tally is betraying Shay, twice; by doing what Dr. Cable wants her to do, and by not telling that she has a tracking device in her necklace.  

-She's also betraying Peris by aprouving the promise she made to Peris (doing whatever it takes to become pretty)

-Friendship

-Tally has an amazing friendship with Shay, to who she's actually  giving her trust and she's basically betraying her just to be able to become pretty. I think that Tally doesn't realize how much a friendship should be valued. 

-Discovery 

-Tally is discovering more way to be happy. By happy I mean focusing on becoming only pretty. Because of Dr. Cable, she can now explore more than the "uglies town" . Now she's exploring all the beauties of the world which are her own beauties as well. I think that she's starting to find herself.
