         <description><![CDATA[<p>1)</p><p>Decision error: Flying through the cloud resulting in reduced visibility</p><p><br></p><p>Lack of rest: The mission was during 3 am, under pressure by land crew, not supposed to relaunch, missionitis</p><p><br></p><p>No prior weather briefings to further prepare the pilots.</p><p><br></p><p>Lack of CRM from both parties.</p><p>(Swiss cheese model + Cockpit Gradient)</p><p><br></p><p>2)</p><p>Must always keep safety as our top priority, do not fall prey to missionitis and there is always another day to train; call off the mission if weather is too hazardous</p><p><br></p><p>Ensure sufficient rest; Risk assessment matrix(RAMS) and aircrew self check</p><p><br></p><p>3)</p><p>Lack of RT (Inefficient communication) </p><p>Poor planning; have proper weather briefing before (Abbreviated briefing)</p><p>One of the heli broke off while the other heli stayed on the same altitude &amp; direction.
