There is not a better way to describe what really happens with this topic than the TV serie "Thirteen reasons why", where every single action, even if it looks pretty simple, while they keep happening they are going to create a huge damage that sometimes they can be permanent. 
This serie, describes 13 reasons, where the most irrelevant things start with a terrible tragedy, where everyone is guilty, not because of the lack of interest, but because of the ignorance, intolerance and the lack of empathy at the moment to interact with others. Sometimes people just remember what they listen from others, without taking the time to ask or make sure that the information they have is real, and they end up spreading fake information, and sometimes exagerating it as they think it is convenient, they just talk about others because they want to avoid their reality.
