The Graffiti Wall is fun activity for students and gives you a visual representation of what your students have learned during a unit of study. Cover a part of a wall with white paper and then encourage students to write or draw what they have learned about a topic. Students can jot down facts, write personal opinions, connect their learning to other areas of study, etc. Using the graffiti wall activity partway through a unit provides you with information for further planning of instruction. The key for the students is to provide lots of details. They could turn in their work to the teacher or they could talk with a partner about what they wrote. Great way to observe the students and their responses as you walk around the room. You can also have the students hold up their whiteboards. A glance at the whiteboards will provide you with information regarding student knowledge and understanding. White boards can be used for a multitude of different lessons, even some which require students to label diagrams. 
As students answer questions, they write their responses down on them for you to see.

Great way to see what students understand and think about a topic. Students write in these journals during writing time.

What's great about it? Students are basically taught on day one of class how to raise their hands and when it is appropriate. For them, it's like second nature. 
By the students showing hands, they tell you whether they need to move on from a lesson or need clarification. By their show of hands, you as the teacher are able to see who understands the material and who may need some extra clarification or practice. This is also great for review on a topic, where you are able to see who may need refreshing on a certain topic or unit lesson. 
Downsides? Some students may not raise their hand, or may say they understand the material when they may not. How to fix? Get to know your students and how they are in the classroom.

I love this assessment! Engaging way to figure out from the students what they know and what they may understand about a topic. Use the questions as prompts for class discussions or have students exchange their question sheets with other students in the class to be checked. 
This gives the students the ability to understand what it might be like to be the teacher and also encourages students to provide positive, descriptive feedback to the student who answered their questions

It is a great way to see how your students interact with each other and assess what they already know or learned about a topic.  This requires students to logically think about a topic and understand the processes behind what they are doing. Students become the teachers and really begin to understand the concepts they are learning. What better way to learn information but to teach it to someone else?

There are alot of students who learn by moving around and being active, which requires culturally responsive teaching. Students respond to a question or statement by going into four different corners of the room labeled: strongly agree, agree, disagree, and strongly disagree and explain their choices.. A great way to assess whether a student understands the information and a great way to see what they may or may not think about a topic. Great for discussions.

Give checklists to the students for tasks and assignments so they have a means to assess whether they have completed all the steps in a task or included all the needed information in an assignment. If students are missing information, they can decide on what course of action they need to take to learn the information.

These exit slips are all open ended and do not require a particular answer. They ask what the child already knew, what they learned, and what they might not understand. These kinds of questions are not  "yes or no" kinds of questions. They allow for more in depth answers and more critical thinking and can be perfect answers for essential questions targeted for that lesson. The same could be said for the formative assessment, open ended questions.
