Ensuite, elles se déplacent au domicile des patients. Ce format peut être inclus dans un programme general d'ETP pour proposer des versions "light" et des versions "complètes". Parfois les patients ne sont pas encore dans une démarche de prise en charge poussée : une formation courte durée qui ne demande pas d'investissement trop important de leur part pourrait répondre aux 1er questionnement.
