At 10:55 a.m. a shipment of 500 pallets of bleach was delivered to Guava Warehouse. However while unloading these pallets the forklift driver in charge of the activity accidently damaged a stack of these pallets which left a massive spillage on the warehouse floor. The cleaner was called in to assist in the clean up , however she didn't up signage when she was done cleaning. while walking in the area a few moments after the cleaning was done , Tim slip and fell over.

What measures could be put it place to prevent such accidents from happening in the future?
