In the book we see that he's constantly in his head switching between worlds. He's having this internal struggle and for a while we see that Caden is stuck and doesn't know what to do. As Neal Shusterman quotes, “You see demons in the eyes of the world, and the world sees a bottomless pit in yours.”

Even in the book the point of view changes to 2nd person. this happens because Caden doesn't know how to handle everything when he first arrives to the Hospital. He's so out of it that it's like he's having an out of body experience. when he somewhat gains control again he doesn't remember anything that happened when he was new to the hospital. 

“They all think medicine should be magic, and they become mad at me when it's not.”

Caden is frustrated with his medicine and always rants about it to his doctor at the hospitals.

“The only thing you have for measuring what's real is your mind . . . so what happens when your mind becomes a pathological liar?”

Caden is confused about what's going on and is fighting with his inner demon.

I've see this a lot in videos, movies and other digital media. Like in the movie Inside Out, the girl has all these creatures in her mind being there acting as her emotions. But since then people have been glorifying it and saying that the creatures inside her head are the mental illnesses,such as a personality disorder which is, completely ridiculous.

I remember just a couple years ago I'd hear people shaming others for speaking out about mental illnesses. It made people like me feeling like these illnesses were something to be ashamed of. But now social media has evolved to where there's a big platform for people to share personal experiences and to help others like them; people coming together to be there for others. We see people over come their own worse enemy. It's a poem talking about Mental Illness.

In season 1 we see that he's this monster who will kill anyone if he doesn't get his way. Through out the show we see that he starts to grow as a person and is on the road to beating his own worse enemy. All while his family sticks through  him and stands by his side.
