In Fidelity, if we say yes, we've taken an assessment or used assessment data to inform decisions, will there be another box that opens up for us to share more info about that?  Or do we just report that we did it?  This question is for all fidelity and structural changes - do we check the box and that's the extent of reporting, or will we be asked additional questions depending on what we check.

This is an example of a health equity component that can be measured. Is it possible to add this in? Or, is it in the CDC reporting?
